Combination of anterograde and retrograde separation in robot-assisted radical prostatectomy with maximum preservation of the periprostatic structures(with surgical video)
Objective:To analyze the clinical application value of the combination of anterograde and retrograde separation in robot-assisted radical prostatectomy with maximum preservation of the periprostatic structures.Methods:By searching relevant literatures at home and abroad,the important anatomical structures and their functions around the prostate were systematically expounded,and the related technologies of combination of anterograde and retrograde separation in robot-assisted radical prostatectomy with maximum preservation of the periprostatic structures were summarized.Results:Neurovascular bundle,accessory pudendal arteries,dorsal vascular complex,and periprostatic supporting structures around the prostate(such as puboprostatic ligament,tendinous arch of pelvic fascia,etc.)play an important role in postoperative recovery of urinary continence and sexual function.Conclusion:The combination of anterograde and retrograde separation in robot-assisted radical prostatectomy with maximum preservation of the periprostatic structures has shown favorable results in terms of urinary continence and sexual function,which has certain advantages in clinical application.
