MAKO机器人辅助全髋关节置换术患者预防DVT的护理策略构建及实施效果探讨

目的:探讨MAKO机器人辅助全髋关节置换术患者预防下肢深静脉血栓(DVT)的护理策略构建及实施效果.方法:选取 2022 年 1 月—2023 年 8 月首都医科大学附属北京积水潭医院收治的 120 例行MAKO机器人辅助全髋关节置换术的患者,随机分为对照组(n=60,常规护理)和观察组(n=60,构建预防DVT的护理干预策略),并比较两组患者DVT发生情况及肢体功能康复情况.结果:观察组在术后1周、2周的DVT发生率均低于对照组,差异有统计学意义(P<0.05).观察组在术后第 1 d、2 d、3 d、5 d、7 d的患肢周径差值及患肢疼痛数字等级评定量表(NRS)评分均低于对照组,差异有统计学意义(P<0.05);观察组在术后第1 d、3 d的D-二聚体(D-D)水平、纤维蛋白原(Fbg)水平、血浆黏度、红细胞聚集指数均低于对照组,差异有统计学意义(P<0.05).观察组在出院时、出院 1 个月的Harris评分高于对照组,且观察组在出院时、出院 3 个月的步长、步速均高于对照组,差异有统计学意义(P<0.05).结论:构建MAKO机器人辅助全髋关节置换术患者的围手术期护理策略,可减少术后DVT的发生,减轻患肢肿胀程度,促进患者关节功能恢复,值得临床应用.
Construction and implementation of nursing strategies for DVT prevention in patients undergoing MAKO robot-assisted total hip arthroplasty
Objective:To explore the construction and implementation effect of nursing strategies for deep venous thrombosis(DVT)prevention in patients undergoing MAKO robot-assisted total hip arthroplasty.Methods:120 patients who underwent MAKO robot-assisted total hip arthroplasty in Beijing Jishuitan Hospital Affiliated to Capital Medical University from January 2022 to August 2023 were selected and randomly divided into the control group(n=60,routine nursing care)and the observation group(n=60,Nursing intervention strategies for DVT prevention).The incidence of DVT and the recovery of limb function in the two groups of patients were compared.Results:The incidence of DVT in the observation group was lower than that in the control group at 1 and 2 weeks after surgery,which was statistically significant(P<0.05).The peridiameter difference of the affected limb and the numerical rating scale(NRS)on the 1st,2nd,3rd,5th,and 7th day after surgery in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The level of D-dimer(D-D)and fibrinogen(Fbg),plasma viscosity,and red blood cell aggregation index in the observation group were lower than those in the control group at the 1st and 3rd day after surgery,and the differences were statistically significant(P<0.05).The Harris score of the observation group was higher than that of the control group at discharge and 1 month after discharge,and the step length and pace of the observation group were higher than those of the control group at discharge and three months after discharge,with statistical significance(P<0.05).Conclusion:The construction of perioperative nursing strategy for MAKO robot-assisted total hip arthroplasty could reduce the occurrence of postoperative DVT,alleviate limb swelling,and promote the recovery of joint function of patients,which is worthy of clinical application.
