住院医师规范化培训中OBE理念的虚拟仿真模式血气分析操作教学探索

目的:在住院医师规范化培训过程中,探索以学生为中心、以学生成果为导向、以持续改进为重点,基于OBE理念的血气分析虚拟仿真模式在血气分析技能培训中的应用效果。方法:研究选取南京医科大学第一附属医院泌尿外科2022级专硕研究生以及2023年10至12月在本科室轮转的专硕研究生为研究对象(n=34),所有受试者在此前均未接受过血气分析的教学,并在教学前测评学生的血气分析技术掌握程度。通过随机抽样法将研究对象分为对照组(n=17)与实验组(n=17)。对照组采用传统带教模式,实验组采用基于OBE理念的血气分析虚拟仿真模式的教学方法。教学后采用客观考核评分与主观评分两部分对教学成果进行最终评估。结果:两组前测客观平均分无明显差异(P=0。897),教学后两组客观评分均有明显进步(P<0。001);实验组的后测客观评分(60。23±2。70)、主观评分(17。18±1。94)以及总评分(77。41±2。24)均显著高于对照组(P<0。05)。结论:在血气分析技能培训教学中,通过以学生为中心、以学生成果为导向、以持续改进为重点,基于OBE理念的血气分析虚拟仿真模式可以帮助学生明确学习目标,熟练掌握血气分析操作,还应学会解读检验报告并做出相应诊治。这有利于评估教学成果,提高住院医师规范化培训质量。
Teaching Exploration of Virtual Simulation Model for Blood Gas Analysis Based on OBEin the Process of Standardized Training of Residents
Objective:To explore the application effect of the student-centered,student-result-oriented and continuous improve-ment based OBE concept virtual simulation model in blood gas analysis skills training in the process of standardized training of resi-dents.Methods:This study selected the 2022 professional master students of Urology Department of the First Affiliated Hospital of Nanjing Medical University and the professional master students who rotated in the department from October to December 2023 as the research objects(n=34).All the subjects had not received blood gas analysis teaching before,and the pretest was used to evaluate the students'mastery of blood gas analysis technology before teaching.The subjects were divided into control group(n=17)and experimen-tal group(n=17)by random sampling method.The control group adopted the traditional teaching model,and the experimental group ad-opted the method of student-centered,student-result-oriented and continuous improvement basedoutcome-based virtual simulation teaching model of blood gas analysis.After teaching,the teaching results were evaluated by objective assessment and subjective evalua-tion.Results:There was no significant difference in the objective mean score of blood gas analysis between the two groups(P=0.897).The objective scores of the two groups of students before and after teaching were significantly improved(P<0.001;The objective score(60.23±2.70),subjective score(17.18±1.94)and total score(77.41±2.24)in the experimental group were significantly higher than those in the control group(P<0.05).Conclusions:In the teaching of blood gas analysis skills,through the method of student-cen-tered,student-result-oriented and continuous improvement based on the OBE-concept-based virtual simulation model of blood gas analysis can help students to clarify their learning objectives,master the blood gas analysis operation,and learn to interpret the test re-port and make corresponding diagnosis and treatment.It is conducive to evaluate the teaching results and improve the quality of resi-dent standardized training.
