Application Effect of Meleis Transition Theory Oriented Hospital Discharge Preparation In-tervention in Patients with Autoimmune Liver Disease
Objective:To explore the application effect of discharge preparation intervention guided by Meleis transition theory in patients with autoimmune liver disease(AILD).Methods:92 AILD patients who were diagnosed and treated in our hospital from Janu-ary 2021 to December 2022 were selected.The method was random number table method,and they were divided into two groups,each with 46 cases.The control group received routine nursing intervention,while the study group received Meleis transition theory oriented discharge preparation intervention.The out of hospital readiness assessment scale(RHDS)and Chinese version of discharge guidance quality scale(QDTS)scores and total scores on the day of discharge were compared between the two groups,The scores and total scores of each dimension of the Chinese version of the Health Promotion Strategy Scale(SUPPH)before and after intervention,as well as pa-tient nursing satisfaction.Result:The RHDS scores of each dimension(disease knowledge,personal status,post-discharge response ability,available social support)and total score were significantly higher compared with the control group(P<0.05).Compared of the teaching skills,content required,content actually obtained,and QDTS total score were significantly higher than the control group(P<0.05).After the intervention,the positive attitude score,stress reduction score,decision score and SUPPH total score were higher(P<0.05),and the study group was significantly higher(P<0.05).The total satisfaction of care in the study group was 100.00%,which was significantly higher than 84.78%in the control group(P<0.05).Conclusion:The Meleis transition theory-oriented discharge readiness intervention for patients with AILD is effective in improving discharge readiness,quality of discharge guidance,self-manage-ment ability,and patient satisfaction.
