DRGs付费背景下促进卒中中心合理用药管理

摘要
以2022年6月-2023年12月卒中中心收治并纳入BR23病组的资料为数据来源,通过医院多个职能部门的联合干预,采用FOCUS-PDCA工具优化合理用药和DRGs相关指标.临床药师主要在合理用药宣讲、参与临床路径管理、医嘱审核、病例点评、药学服务、审查超支DRGs病组用药方案等方面进行合理用药管理.结果显示,与干预前相比,干预后的用药品种数、药占比、重点监控品种金额占比、住院天数、DRGs超支率、超支住院费用均下降(P<0.05),国家基本药物金额占比提高(P<0.05).
Abstract
Based on the data from cases admitted to the stroke center and included in the BR23 dis-ease group from June 2022 to December 2023,through collaborative interventions from various hospital de-partments,the FOCUS-PDCA tool was employed to optimize rational drug use and DRGs-related indicators.Clinical pharmacists mainly carried out rational drug use management through activities such as drug use advocacy,participation in clinical pathways management,prescription review,case evaluations,pharmaceuti-cal services and reviewing drug plans for DRGs with excessive expenditures.The results showed that com-pared to the pre-intervention period,the post-intervention period achieved decreases in the number of drug categories,drug expenditure ratio,proportion of expenditure on monitored drug categories,hospitalization day,DRGs overpayment rate,and overpayment inpatient expense(P<0.05);while the proportion of national essential drug expenditure increased(P<0.05).
