超声斑点追踪成像技术对T2DM伴高血压患者左心室舒张功能的评估价值

目的 探讨超声斑点追踪成像技术(STI)对2型糖尿病(T2DM)伴高血压患者左心室舒张功能的评估价值.方法 回顾性分析135例单纯高血压患者(EH组)、105例单纯T2DM患者(T2DM组)、107例高血压合并T2DM患者(T2DM+EH组)及33例门诊健康体检者(NC组)临床资料.四组均行常规超声心动图检查,测量室间隔厚度(IVST)和左心室后壁厚度(LVPWT),计算左心室质量指数(LVMI)和相对室壁厚度(RWT).运用EchoPaC超声工作站进行脱机分析,测量解旋减半时间(HTU)、解旋速度峰值(PUV)和解旋速度达峰时间(TPUV),计算等容解旋比例(IUR)和解旋率(UntwR).结果 T2DM+EH 组 LVPWT、IVST、RWT 和 LVMI 均高于 NC 组、EH组和 T2DM 组(P<0.05);EH 组 LVPWT、RWT 和 LVMI 均高于 T2DM 组和 NC 组(P<0.05).T2DM+EH 组 HTU、PUV 和 TPUV 均高于 EH 组、T2DM 组和 NC 组(P<0.05),而IUR和 UntwR 低于 EH 组、T2DM 组和 NC 组(P<0.05).EH 组 HTU、PUV 和 TPUV 高于T2DM 组和 NC 组(P<0.05),而 IUR 和 UntwR 低于 T2DM 组和 NC 组(P<0.05).T2DM 组HTU、PUV 和 TPUV 高于 NC 组(P<0.05),而 IUR 和 UntwR 低于 NC 组(P<0.05).结论 T2DM合并高血压时可引起更严重的左心室肥厚和左心室舒张功能不全.STI技术可以定量、无创、重复分析患者左心室解旋运动.解旋动力学参数可为评价左心室舒张功能提供新的量化指标.
Value of ultrasound speckle tracking imaging in evaluation of left ventricular diastolic function in T2DM patients with hypertension
Objective To explore the value of ultrasound speckle tracking imaging(STI)in evaluating the left ventricular diastolic function of patients with type 2 diabetes mellitus(T2DM)combined with hypertension.Methods The clinical data of 135 patients with simple hypertension(EH group),105 patients with simple T2DM(T2DM group),107 patients with hypertension combined with T2DM(T2DM+EH group)and 33 outpatients underwent physical examination(NC group)were analyzed retrospectively.The interventricular septal thickness(IVST),left ventricular posterior wall thickness(LVPWT),left ventricular mass index(LVMI)and relative wall thickness(RWT)of the four groups were measured by routine echocardiography.The off-line analysis was carried out using the EchoPaC ultrasonic station.The half time of untwisting(HTU),peak untwisting velocity(PUV)and time to peak untwisting velocity(TPUV)were measured,and the isovolumic untwisting rate(IUR)and untwisting rate(UntwR)were calculated.Results The LVPWT,IVST,RWT and LVMI in T2DM+EH group were higher than those in groups of NC,EH and T2DM(P<0.05).The LVPWT,RWT and LVMI in EH group were higher than those in groups of T2DM and NC(P<0.05).The HTU,PUV and TPUV in T2DM+EH group were higher than those in groups of EH,T2DM and NC(P<0.05),while IUR and UntwR were lower than those in groups of EH,T2DM and NC(P<0.05).The HTU,PUV and TPUV in EH group were higher than those in groups of T2DM and NC(P<0.05),while IUR and UntwR were lower than those in groups of T2DM and NC(P<0.05).The HTU,PUV and TPUV in T2DM group were higher than those in NC group(P<0.05),while IUR and UntwR were lower than those in NC group(P<0.05).Conclusion T2DM combined with hypertension can cause more serious left ventricular hypertrophy and left ventricular diastolic dysfunction.STI technique can be used to quantitatively,noninvasively and repeatedly analyze left ventricular untwisting motion in patients.The parameters of untwisting dynamics may provide a new quantitative index for evaluating the left ventricular diastolic function.
