Effect of maximal preservation of functional urethra during transurethral holmium laser enucleation of prostate on postoperative stress urinary incontinence in patients with benign prostatic hyperplasia
Objective To explore the effect of maximal preservation of functional urethra during transurethral holmium laser enucleation of prostate(HoLEP)on postoperative stress urinary incontinence(SUI)in the patients with benign prostatic hyperplasia(BPH).Methods Among 114 BPH patients,57 patients underwent modified transurethral HoLEP with maximal preservation of functional urethra(group A),and 57 patients underwent traditional transurethral HoLEP(group B).Perioperative indicators were compared between the two groups.The occurrence of SUI immediately after extubation and in the 1st,2nd and 4th week after surgery were recorded.The indicators of International Prostate Symptom Score(IPSS),quality of life(QoL)score,maximum urine flow rate(Qmax)and residual urine volume(RUV)before and in the 1st and 6th month after surgery were evaluated.Results The surgery of all BPH patients was completed successfully.The perioperative indicators were similar between two groups(P>0.05).Compared with group B,the incidence rates of SUI immediately after extubation and in the 1st and 2nd week after surgery were decreased in group A(P<0.05 or P<0.01).Compared with before surgery,IPSS,QoL score and RUV were decreased and Qmax was increased in the 1st and 6th month after surgery in two groups(P<0.01),while IPSS,QoL score,Qmax and RUV in the 1st and 6th month after surgery in two groups were similar(P>0.05).Conclusion Maximal preservation of functional urethra during transurethral HoLEP can effectively reduce the occurrence of postoperative SUI in BPH patients.
