Expression and clinical significance of serum irisin in maintenance hemodialysis patients combined with sarcopenia
Objective To investigate the expression and significance of serum irisin in maintenance hemodialysis(MHD)patients combined with sarcopenia. Methods The grip strength was evaluated in 100 patients with MHD,in whom the fat tissue index(FTI)and lean tissue index(LTI)were detected by body composition analyzer.According to the diagnostic criteria of the Asian Sarcopenia Working Group,100 MHD patients were divided into sarcopenia group(53 cases)and non-sarcopenia group(47 cases). The clinical data of the two groups were compared.Serum irisin level was detected by ELISA assay. The correlation between serum irisin level and other clinical indexes was analyzed, and the risk factors of MHD combined with sarcopenia were analyzed. Results There were no statistically significant differences in age, dialysis age, male ratio,BMI,upper arm circumference,FTI,TC,blood calcium and parathyroid hormone between the two groups(P>0.05).The walking speed, grip strength,LTI and serum irisin level were lower, and the level of SCr was higher in sarcopenia group than those in non-sarcopenia group(P<0.05).Spearman correlation analysis showed that serum irisin level was negatively correlated with SCr level(P<0.05),and positively correlated with LTI(P<0.05).Multivariate logistic regression analysis showed that higher serum irisin level was the independent protective factor for MHD combined with sarcopenia(P<0.05).Conclusion Serum level of irisin in MHD patients combined with sarcopenia is lower.Higher serum irisin level is the protective factor for MHD combined with sarcopenia.
