An analysis of influencing factors for ischemic cardiomyopathy in patients with heart failure with reduced ejection fraction
Objective To analyze the influencing factors for ischemic cardiomyopathy(ICM)in the patients with heart failure with reduced ejection fraction(HFrEF).Methods Of 254 patients with HFrEF,129 cases were combined with ICM(group A),and 125 cases were not combined with ICM(group B).The basic clinical data and indicators of laboratory examination,ultrasonic cardiogram and electrocardiogram were compared between groups of A and B.The independent influencing factors for ICM in the patients with HFrEF were analyzed by multivariate logistic regression.Results The constituent ratio of gender,age,heart function classification and percentages of hypertension and diabetes mellitus in group A were significantly different from those in group B(P<0.05).Compared with group B,the level of uric acid,left atrial diameter,left ventricular end-diastolic dimension,QRS duration and percentages of atrial fibrillation or flutter,high voltage in left ventricle and left bundle branch block were decreased,while the level of high-sensitivity troponin T,left ventricular ejection fraction and percentages of regional wall motion abnormality(RWMA),ST-T changes,pathological Q wave and T wave polarity in lead aVR were increased in group A(P<0.05).The multivariate logistic regression analysis showed that having hypertension,diabetes mellitus,RWMA,pathological Q wave and T wave polarity in lead aVR were the independent risk factors for ICM in the patients with HFrEF(P<0.05),while having atrial fibrillation or flutter was the independent protective factor(P<0.05).Conclusion Having hypertension,diabetes mellitus,RWMA,pathological Q wave and T wave polarity in lead aVR are the independent risk factors for ICM in the patients with HFrEF.
