Objective:To develop a case management information system for pulmonary rehabilitation outpatient clinics based on decision support system and hospital HIS system,and to establish a continuous monitoring electronic file for patients who receive full management of pulmonary rehabilitation.Methods:The nursing department,the pulmonary rehabilitation clinic and the information center shall carry out the whole-process pulmonary rehabilitation case management according to the needs of the clinic.Results:The whole process case management information system including outpatient case receiving,evaluation,consultation,treatment,follow-up and case closing was constructed.After the application of the system,case managers had a high evaluation on the practicability,data reliability,decision support and overall satisfaction of the system.Conclusions:Case management information system can provide scientific,whole-process and decision support for case managers,help them to carry out outpatient case management more efficiently,and promote case management from"manual document management"stage to"data information management"stage.
