目的 探讨膝骨关节炎(KOA)与肌肉减少症(肌少症)的相关性.方法 计算机检索Medline(via PubMed)、Embase、Web of ScienceACochrane Library、中国知网、万方数据库建库至2023年2月有关KOA患者肌少症的研究文献.2名评审员利用Endnote 20软件独立完成筛选文献及资料提取.采用State15.0软件对KOA与肌少症的相关性进行Meta分析,主要结果定义为KOA中肌少症的患病率.结果 KOA中肌少症患病率为29.4%(95%CI:0.177~0.410).进一步亚组分析发现男性KOA患者肌少症患病率为35.3%(95%CI:0.051~0.654),略低于女性的37.3%(95%CI:0.164~0.583);80岁以上KOA患者肌少症患病率最高,为71.8%(95%CI:0.404~1.032);亚洲国家KOA患者肌少症患病率最高,为32.4%(95%CI:0.175~0.473).横断面研究的KOA患者肌少症患病率为33.1%(95%CI:0.195~0.468),明显高于病例对照研究的5.5%(95%CI:-0.013~0.122).结论 KOA患者中肌少症的患病率较高,且患病率受年龄的影响较大.应重视KOA患者肌肉的质量和功能,采取适当的肌肉力量训练,并加强蛋白质摄入,以减缓膝骨关节炎的发展.
Meta-analysis on the Association between Knee Osteoarthritis and Sarcopenia
Objective To investigate the association between knee osteoarthritis(KOA)and sarcopenia by meta-analysis.Methods We systematically review the literature from 1985 to 2023,including cross-sectional studies and case-control studies.All articles have been evaluated by two independent reviewers.We performed a meta-analysis with the primary outcome defined as the prevalence of sarcopenia in knee osteoarthritis(KOA).Results The detection rate of sarcopenia in knee osteoarthritis patients was 29.4%(95%confidence interval,0.177-0.410),and the detection rate of sarcopenia in male KOA patients in subgroup analysis was 35.3%(0.051-0.654),lower than the detection rate of 37.3%(95%CI:0.164-0.583)for women.The detection rate of sarcope-nia was highest in KOA patients over 80 years of age,71.8%(95%CI:0.404-1.032);the detec-tion rate of sarcopenia was highest in KOA patients in Asian countries,32.4%(95%CI:0.175-0.473).The detection rate of sarcopenia in KOA patients in cross-sectional studies was 33.1%(0.195-0.468),which was significantly higher than that in the case-control study of 5.5%(95%CI:-0.013-0.122).Conclusion Meta-analysis revealed that the detection rate of sarcopenia is higher in KOA patients,and the age of patients has a greater impact on the prevalence.Therefore,clinically,patients with osteoarthritis should pay attention to the quality and function of their muscles,perform appropriate strength training for related muscles,and at the same time strength-en protein intake to reduce inflammation,so as to slow down the development of knee osteoarthritis.
