中医综合疗法联合关节腔灌注治疗冻结肩疗效观察

摘要

目的:观察中医综合疗法联合关节腔灌注治疗冻结肩的临床疗效.方法:将 120 例冻结肩患者随机分为常规治疗组、中医综合治疗组及联合治疗组,各 40 例.常规治疗组给予口服艾瑞昔布片配合常规功能锻炼;中医综合治疗组给予中医推拿、当归四逆汤口服及常规功能锻炼;联合治疗组予中医综合治疗配合肩关节灌注治疗.各组疗程均为 4 周.在治疗前及治疗 6 周后分别行CRP、PGE2 水平检测,中文版Constant-Murley肩关节功能评分(CMS评分)以及临床治疗效果评定.结果:3 组患者治疗后较治疗前CRP、PGE2 水平均有下降(P<0.01),CMS评分均上升(P<0.01);在CRP、PGE2 水平及CMS评分方面,中医综合治疗组优于常规治疗组(P<0.01),联合治疗组治疗后较中医综合治疗组改善更明显(P<0.01);常规治疗组和中医综合治疗组总有效率比较无统计学意义(P>0.05),但联合治疗组总有效率明显优于常规治疗组(P<0.01)和中医综合治疗组(P<0.01).各组治疗过程中无不良反应出现.结论:中医综合疗法联合关节腔灌注治疗冻结者可减轻炎症反应,缓解疼痛并改善肩关节功能,优于常规治疗及中医综合疗法.

Abstract

Objective:To evaluate the efficacy of TCM combined with intra-articular perfusion in the treatment of frozen shoulder.Methods:One hundred and twenty patients with frozen shoulder were randomly divided into conventional treatment group,TCM comprehensive treatment group and combined treatment group,40 cases in each group.The routine treatment group was given oral administration of irexib tablets combined with routine functional exercise;The TCM comprehensive treatment group was given TCM Massage,Danggui Sini Decoction and routine functional exercise.The combined treatment group was treated with TCM combined with shoulder joint perfusion.The treatment course of each group was 4 weeks.CRP and PGE2 levels,Chinese version of Constant-Murley shoulder function score(CMS)and clinical effect were measured before and 6 weeks after treatment.Results:After treatment,the levels of CRP and PGE2 in three groups were decreased(P<0.01),and the CMS scores were increased(P<0.01);in terms of CRP,PGE2 levels and CMS scores,the comprehensive treatment group was better than the conventional treatment group(P<0.01),and the combined treatment group was better than the comprehensive treatment group(P<0.01).There was no significant difference in the total effective rate between the conventional treatment group and the TCM Comprehensive treatment group(P>0.05),but the total effective rate of the combined treatment group was significantly better than that of the conventional treatment group(P<0.01)and the TCM Comprehensive treatment group(P<0.01).There was no significant difference in the total effective rate between the routine treatment group and the comprehensive treatment group of traditional Chinese medicine(P>0.05).There were no adverse reactions in each group.Conclusion:TCM comprehensive therapy combined with intra-articular perfusion can reduce inflammatory reaction,relieve pain and improve shoulder joint function,which is superior to conventional treatment group and TCM comprehensive therapy.
