Evaluation of the effects of orthodontic treatment on alveolar bone,CGRP level and microenvironment remodeling in patients with anterior tooth displacement caused by periodontal disease based on generalized estimation equation
Objective To evaluate the effects of orthodontic treatment on alveolar bone,calcitonin gene-related peptide(CGRP)levels and microenvironment remodeling in patients with anterior tooth displacement caused by periodontal disease by generalized estimation equation.Methods A total of 286 patients with anterior tooth displacement due to periodontal disease who were treated in the hospital from August 2019 to August 2022 were selected,and divided into the control group(n=143,receiving periodontal basic treatment)and the observa-tion group(n=143,receiving orthodontic treatment)according to random number table method.Clinical data,CGRP level,anterior tooth covering distance,periodontal pocket depth,alveolar bone height,bleeding index,tu-mor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-2(IL-2)before and after treatment were compared between the two groups.Generalized estimation equation(GEE)was constructed to analyze the relationship be-tween CGRP level and remodeling of alveolar bone and microenvironment after orthodontic treatment.The correla-tion between CGRP level and anterior tooth covering distance,periodontal pocket depth,alveolar bone height,TNF-α,IL-6 and IL-2 levels was analyzed using Spearman's method.Results Compared with the pre-treatment period,the patients with anterior tooth displacement caused by periodontal disease in the two groups showed an in-crease of CGRP level,anterior tooth covering reduced,periodontal pocket depth decreased,alveolar bone height de-creased,bleeding index decreased,and secretion of inflammatory factors TNF-α,IL-6 and IL-2 decreased.The above indexes of patients with anterior tooth displacement in the observation group were significantly superior to those in the control group(P<0.05).Spearman correlation analysis showed that serum CGRP was significantly negatively correlated with anterior tooth coverage distance,periodontal pocket depth,alveolar bone height,bleed-ing index,TNF-α,IL-6,and IL-2(P<0.05).GEE analysis results showed that the anterior tooth covering distance,periodontal pocket depth,alveolar bone height,bleeding index,TNF-α,IL-6 and IL-2 in the observation group were superior to those in the control group after treatment,and the difference was statistically significant(P<0.05).Conclusion The level of CGRP is closely correlated with the remodeling of alveolar bone and microenvironment in patients with periodontal disease-induced anterior tooth displacement during orthodontic treatment.It is possible to evaluate the treatment effect of patients based on the correlation between CGRP levels,and the alveolar bone and microenvironmental modification in orthodontic treatment of patients with anterior tooth displacement caused by periodontal disease,and plays an important role in dental treatment.
