Objective:To investigate the imaging manifestations of cone-beam CT in patients with temporomandibular joint osteoarthritis(TMJOA)who are sensitive to sudden temperature drop.Methods:97 patients with TMJOA(194 lateral joints),who attended the Department of Stomatology of the First Medical Center of the General Hospital of the People's Liberation Army(PLA)from December 2022 to December 2023 were included.TMJOA patients were divided into sensitive and insensitive groups by questionnaire.The clinical symptoms of the two groups were observed,and the cone-beam CT images of their condylar bones were analyzed.SPSS 25.0 software was used for statistical analysis.Results:There was no statistically significant difference between the gender and age of the patients in the temperature drop-sensitive group(47 patients)and the insensitivity group(50 patients)(P>0.05).The percentage of patients with painful symptoms was higher in the temperature drop-sensitive group[93.6%(44/47)]than that in the insensitive group[70.0%(35/50)](x2=8.941,P=0.003),and there was no statistically significant difference in joint murmur and mouth opening restriction between the two groups.Comparison of the condylar bone images of the 194 lateral joints showed that the condylar bone was predominantly absorbed and destroyed in the temperature-drop-sensitive group,with a significantly higher proportion of surface bone destruction[77.7%(73/94)]and condylar flattening and shortening[53.2%(50/94)]than that in the non-sensitive group(64.0%(64/100)](x2=4.357,P=0.037).The proportion of condylar flattening and shortening[38.0%(38/100)](x2=4.512,P=0.034),and cystic degeneration[19.1%(18/94)]were also significantly higher than that of the insensitive group[9.0%(9/100)](x2=4.166,P=0.041).The depth of joint fossa IAE was greater in the temperature drop-sensitive group than in the temperature plunge-insensitive group(P=0.033),and the joint elevation tilt DGF was also significantly greater in the temperature drop-insensitive group(P=0.014).There were significant differences in the 120 and 150 joint gaps between the two groups,with the 120 joint gap in the temperature-sensitive group being smaller than that in the insensitive group(P=0.043)and the 150 joint gap in the temperature-sensitive group being smaller than that in the insensitive group(P=0.029).Conclusion:Patients with TMJOA in which the condylar bone is resorption-destructive are more likely to be sensitive to temperature drop,and temperature drop may be one of the factors promoting the progression of TMJOA,resulting in resorption-destruction of the condylar bone and posterior displacement of the condyle.
