Early application of lengthening PFNA internal fixation for senile subtrochanteric fracture of femur Clinical study of Ibandronate sodium in the treatment of osteoporosis
Objective To investigate the effect of early postoperative application of Ibandronate sodium on reducing postoperative osteoporosis in elderly patients with subtrochanteric fracture of femur who received lengthening intramedullary nail (PFNA) internal fixation. Methods 67 patients with subtrochanteric fracture of femur who received extended PFNA internal fixation from January 2017 to July 2018 were selected and randomly divided into experimental group (37 cases) and control group (30 cases). The patients were given 2 mg of ibandronate sodium (250 mL) and normal saline (250 mL) 2 weeks after surgery, respectively. Patients in both groups were given calcitci D 600 mg for a long time. Fracture healing time, 3-month Harris hip score, 6-month and 1-year changes in bone density values of the contralateral hip, and incidence of refracture were compared between the two groups. Results A total of 64 patients completed 1 year follow-up, the fracture healing time of the experimental group was (11.08±1.723) weeks, the fracture healing time of the control group was (12.08±1.513) weeks, the difference was statistically significant (P>0.05). Three months after surgery, the Harris score of hip joint in the experimental group was (85.15±3.71) points, and the Harris score of contralateral hip joint in the control group was (67.58±3.49) points, the difference was statistically significant (P<0.05). Six months after surgery, the bone mineral density of the contralateral hip joint of the experimental group was (-2.58±3.723) g/cm2, and that of the control group was (-5.58±3.423) g/cm2, the difference was statistically significant (P<0.05). One year after surgery, the bone density of the contralateral hip joint in the experimental group was (1.58±3.323) g/cm2, and that in the control group was (-5.38±2.423) g/cm2, the difference was statistically significant (P<0.05). Fracture nonunion occurred in 1 case in control group 1 year after operation. Conclusion Early application of ibandronate sodium after extended PFNA internal fixation for senile femoral subtrochanteric fractures can treat osteoporosis and shorten fracture healing time to a certain extent.
