The continuous health management model of chronic disease will become an effective means to overcome the difficulties in the current diagnosis and treatment of chronic disease, such as the loss of follow-up, referral difficulties and separation of medical treatment and prevention, and has shown considerable potential in the early operation, and has been widely welcomed by medical staff and patients with chronic disease, and produced a good response. However, the chronic disease continuous health management model is still in a process of continuous development and innovation, so it also faces many problems.
