Application of a diabetes specialist nurse-led family physician contracting service model in the management of diabetic patients in the community
Objective To explore the application effect of diabetes specialist nurse-led family doctor contracting service model in the management of diabetic patients in the community.Methods From January 2022 to December 2022,192 diabetic patients in the diabetes health management file of a town in Jiangyin City were conveniently selected as the subjects of this study,and were divided into two groups of 96 cases each.The contracting service for the control group was provided by a traditional family doctor team,and the contracting service for the observation group was provided by a family doctor team led by a diabetes specialist nurse.After one year of contracted services,the capillary blood glucose values,DSQL scores(a specific quality of life scale for diabetes patients),and satisfaction with contracted services of diabetic patients in the 2 groups were compared.Results The difference in blood glucose 2h after breakfast was not statistically significant between the two groups(P>0.05),and the difference in fasting blood glucose,blood glucose 2h after lunch,blood glucose 2h after dinner,randomized blood glucose,DSQL scores,and satisfaction with contracted services between the two groups was statistically significant(P<0.01).Conclusion Diabetes specialist nurse-led family doctor contracting service model can effectively help community diabetic patients control their blood glucose level,improve their quality of life and survival,and increase their satisfaction with contracted services.
