摘要

目的 探讨面神经血管瘤的高分辨率CT(HRCT)和MRI表现,以提高其影像诊断水平.方法 回顾性分析8例面神经血管瘤患者的HRCT及MRI资料并与手术及病理结果对照.结果 病变发生于左侧5例,右侧3例.HRCT像上表现为膝状神经窝扩大、增宽伴局部密度增高(8/8),其中5例病灶内可见“点”状及“针”状高密度区,7例同时伴有迷路段增宽,4例水平段增宽.病灶边界欠清楚,周围无骨质硬化表现.MRI上平扫T1WI接近等信号、T2WI上信号稍高,增强扫描均呈较明显强化,5例不均匀强化,3例强化较均匀.结论 面神经血管瘤的影像学表现具有一定特征性.HRCT与MRI结合有助于全面评价,可早期诊断为治疗提供依据.

Abstract

Objective To investigate HRCT, MRI findings of facial nerve hemangioma in order to improve the diagnostic accuracy. Methods The HRCT,MRI images of 8 patients with pathologically proved facial nerve hemangioma were e-valuated retrospectively,the imaging findings were compared with the operative and pathological findings. Results 5 lesions were located in the left side and 3 lesions in right. On HRCT imaging, all the cases demonstrated slight hyperdense masses in the enlarged geniculate fossa of facial nerve canal, small flecks or spicules of calcification were observed in 5 cases. 7 cases demonstrated enlargement of labyrinth segment at the same time,and mastoid segment enlarged in 4 cases. The margins of lesions were indistinct and there was no sclerotic margin. On MR imaging, the lesions showed isointensity on T1 WI , slight hyperintensity on T2WI. After administration of contrast medium, all the lesions enhanced markedly ,5 lesions were homogeneous enhancement and 3 lesions appeared heterogeneous enhancement. Conclusion Imaging findings of facial nerve hemangioma have some characteristics. HRCT combinated with MRI are helpful in early diagnosis, which may contribute to the treatment.
