经颅重复针刺刺激疗法对慢性疲劳综合征脑功能调控的fMRI研究

目的 通过静息态功能磁共振(rs-fMRI)技术记录脑功能信息,使用磁共振局部功能活动特性分析方法比率低频振幅(fALFF),研究经颅重复针刺刺激疗法(rTAS)对慢性疲劳综合征(CFS)患者自发脑活动的调控作用及其与临床症状改善之间的相关性.方法 在2021年3月至2022年3月期间,面向在读研究生招募符合CFS诊断标准的患病被试20例,纳入rTAS治疗组.对该组患者实施rTAS治疗8周,治疗前、后对所有被试进行症状学量表测评及静息态功能磁共振(rs-fMRI)扫描,评价rTAS对CFS的疗效,并使用fALFF分析比较CFS患者脑功能治疗前后的变化情况.结果 经过8周治疗,CFS患者症状相关量表评分结果均有显著变化,其中疲劳量表FS-14、症状自评量表(SCL-90)及匹兹堡睡眠质量指数量表(PSQI)评分降低(P<0.05),蒙特利尔认知量表(MoCa)评分升高(P<0.05);此外,左侧中央后回fALFF升高,右侧小脑上部fALFF降低;且以上差异脑区的fALFF值与治疗前后临床症状的变化存在一定相关,左侧中央后回fALFF值与FS-14、SCL-90、PSQI存在负相关(均P<0.05),与Mo-Ca 存在正相关(P<0.05);右侧小脑上部fALFF值与FS-14、PSQI存在负相关(均P<0.05),与MoCa存在正相关(P<0.05).结论 rTAS能改善CFS患者临床症状,改变患者脑功能的自发活动状态,CFS患者治疗前后的fALFF改变与临床症状相关,提示rTAS治疗CFS是通过中枢神经调控机制实现的.
Objective Brain function information was recorded by rs-fMRI,and fALFF was used to analyze the local functional activity characteristics of magnetic resonance,to study the regulative effect of rTAS on spontaneous brain activity in patients with CFS and its correlation with the improvement of clinical symptoms.Methods From March 2021 to March 2022,20 patients who met the diagnostic criteria for CFS were recruited for postgraduate students majoring in clinical medi-cine,then they were included in the rTAS treatment group.The patients in this group were treated with rTAS for 8 weeks.Before and after treatment,all subjects were evaluated with symptom scale and rs-fMRI scanning to evaluate the effect of rTAS on CFS,and fALFF was used to analyze and compare the changes of brain function in CFS patients before and after treatment.Results After 8 weeks of treatment,CFS patients had significant changes in symptom related scale scores,in-cluding FS-14,SCL-90 and PSQI scale scores decreased(P<0.05),and MoCa scores increased(P<0.05).In addition,the fALFF in the left posterior central gyrus increased,while that in the right upper cerebellum decreased.In addition,the fALFF values in the brain regions with the above differences had certain correlation with the changes of clinical symptoms before and after treatment.The fALFF values in the left posterior central gyrus had negative correlation with FS-14,SCL-90,PSQI(all P<0.05),and had positive correlation with MoCa(P<0.05).The fALFF value in the right upper cerebel-lum was negatively correlated with FS-14 and PSQI(all P<0.05),and positively correlated with MoCa(P<0.05).Conclusion The rTAS can improve the clinical symptoms of CFS patients and change the spontaneous activity of brain function.The changes of fALFF in CFS patients before and after treatment are related to the clinical symptoms,suggesting that rTAS treatment of CFS is achieved through the central nervous regulatory mechanism.
