Objective To quantitatively measure the changes of gray matter nuclei in patients with unilateral acute is-chemic stroke by quantitative magnetic susceptibility imaging(QSM).Methods 49 patients with unilateral acute ischemic stroke with the first onset were collected,all patients underwent routine head MRI and QSM examination within 7 days of symptom onset,and magnetic sensitive images were obtained by post-processing software,and RO1 was manually delineated on the magnetic sensitivity map,including globuspallidus,putamen,caudate nucleus head and thalamus,and bilateral mag-netic susceptibility values were measured.According to the size of the lesions displayed on DWI,the patients were divided into lacunar infarction group,small infarct group and large group,and the difference in bilateral magnetic susceptibility difference of each ROI between the three groups was compared by one-way ANOVA.Results Among 49 patients with a-cute ischemic stroke,the magnetic susceptibility values of globus pallidus,putamen and caudate nucleus head were signifi-cantly different between the stroke side and the unaffected side(P<0.05),while the magnetic susceptibility values of the thalamus on both sides of the subjects were not significantly different(P>0.05).In the above three groups,the difference between the two sides of the globus pallidus magnetic susceptibility was statistically significant(F=5.471,P=0.007),and the difference in bilateral susceptibility difference between the small infarct group and the large infarct group was statistical-ly significant compared with the lacunar infarct group(P<0.05).There was no significant difference among the other puta-men,caudate head and thalamus groups(P>0.05).Conclusion In patients with acute ischemic stroke,the level of brain iron in some brain regions on the side of stroke in the early stage of onset is higher than on the healthy side,and it was found in the globus pallidus region,when the infarct was larger than 1.5 cm,the iron deposition on the stroke side was significantly increased compared with the healthy side.Therefore,Brain iron deposition may be a risk factor for the treatment of ischemic stroke.The regulation of brain iron level can be an important target for clinical treatment and provide guidance for progno-sis.
