Correlation Between the Overall Burden of Small Cerebral Vascular Disease and Coronary Atherosclerosis

目的 探讨脑小血管病(CSVD)冠状动脉粥样硬化相关指标与CSVD总体负担的相关性.方法 搜集行冠状动脉CTA检查、头颅MRI检查及颈动脉超声检查的CSVD患者,根据CSVD总体负担评分将研究对象分为0分组、1 分组、2 分组及3~4 分组.基于CCTA图像获得冠状动脉粥样硬化相关指标:包括Gensini评分、冠状动脉钙化积分(CACS)、冠状动脉3 条主要分支狭窄最明显处的狭窄程度及CT血流储备分数(FFR)、3 支冠状动脉近端周围脂肪衰减指数(FAI)等.分析各指标与CSVD总体负担之间的相关性.进一步将所有研究对象分为高分组(3~4 分)及低分组(0~2 分),分析确定CSVD总体负担高评分的有效预测因子,并评价各模型对CSVD总体负担高评分的预测效能.结果 最终纳入169 例CSVD 患者,包括CSVD评分0 分组45 例,1 分组43 例,2 分组41 例,3~4 分组40 例.Spearman相关性分析显示年龄、高血压分级、总斑块数、最大狭窄程度、Gensini评分、CACS与CS-VD负担评分呈正相关(rs>0.3,P<0.05),FFR-最小与CSVD负担评分呈负相关(rs =-0.464,P<0.05).根据CSVD总体负担评分将研究对象分为高分组(3~4 分)和低分组(0~2 分),高分组 40 例,低分组 129 例.二元Lo-gistic回归分析显示高血压分级(OR =2.367,P =0.001)及FFR-最小(OR =0.916,P =0.033)为CSVD总体高负担评分的有效预测因子.ROC曲线分析显示,对CSVD总体高负担评分发生风险预测效能的曲线下面积(AUC)从大到小依次是高血压分级和 FFR-最小的联合模型(AUC =0.811,P<0.001),高血压分级(AUC =0.759,P<0.001)以及FFR-最小(AUC =0.752,P<0.001).结论 CSVD患者冠状动脉粥样硬化与CSVD总体负担具有一定相关性:冠状动脉总斑块数、最大狭窄程度、Gensini评分、CACS与CSVD总体负担评分中度正相关,而FFR-最小与CS-VD总体负担评分呈中度负相关;高血压分级及FFR-最小为CSVD总体高负担评分患者的有效预测因子,可有助于临床更好地对高风险CSVD患者进行早期干预.
Objective To explore the correlation between coronary atherosclerosis related indicators and the overall bur-den of small cerebral vascular disease(CSVD).Methods Collect CSVD patients who underwent coronary CTA,head MRI and carotid ultrasound examination,and divide the subjects into groups0,1,2 and3-4 according to the overall burden score of CSVD.Based on the CCTA image,the coronary atherosclerosis related indicators were obtained,including Gensini score,coronary artery calcification score(CACS),the degree of stenosis and CT blood flow reserve score(FFRCT)at the most obvious stenosis of the three main coronary branches,and the fat attenuation index(FAI)around the proximal end of the three coronary arteries.Analyzed the correlation between the indicators and the overall burden of CSVD.All subjects were further divided into high group(3-4 points)and low group(0-2 points)to analyze and determine the effective pre-dictors of the high overall burden score of CSVD,and evaluate the predictive effectiveness of each model on the high overall burden score of CSVD.Results 1.169 patients with CSVD were finally included,including 45 patients with CSVD score 0,43 patients with CSVD score1,41 patients with CSVD score2,and40 patients with CSVD score3-4.Spearman correla-tion analysis showed that age,hypertension grade,total plaque number,maximum stenosis degree,Gensini score,CACS were positive correlated with CSVD burden score(rs =0.406,0.529,0.350,0.380,0.395,0.358,all P<0.05),and and FFR-minimum was negative correlated with CSVD burden score(rs =-0.464,P<0.05).2.According to the overall burden score of CSVD,the subjects were divided into high group(3-4 points)and low group(0-2 points),with40 cases in high group and 129 cases in low group.Binary logistic regression analysis showed that hypertension grade(OR =2.367,95%CI:1.440-3.892,P =0.001)and FFR-minimum(OR =0.916,95%CI:0.845-0.993,P =0.033)were effective pre-dictors of the overall high burden score of CSVD.ROC analysis showed that the area under the curve(AUC)of the com-bined model of hypertension classification and FFR-minimum was the highest(AUC =0.811,95%CI:0.745-0.868,P<0.05)for the risk prediction effectiveness of the overall high burden score of CSVD,followed by hypertension classification(AUC =0.759,95%CI:0.688-0.822,P<0.05),and FFR-minimum(AUC =0.752,95%CI:0.680-0.815,P<0.05).Conclusion In patients with CSVD,there is a certain correlation between coronary atherosclerosis and the overall burden of CSVD:the number of total coronary artery plaque,the degree of maximum stenosis,the Gensini score,CACS and the overall burden of CSVD have a moderate positive correlation,while FFR-minimum and the overall burden of CSVD have a moderate negative correlation;Hypertension grade and FFR-minimum are effective predictors of the overall high burden score of CSVD patients,which can be helpful for early intervention of high-risk patients with CSVD.
