Objective To investigate the diagnostic value of enhanced T2 fluid-attenuated inversion-recovery(FLAIR)-T1 WI mismatch feature in predicting isocitrate dehydrogenase genes(IDH)mutation status in diffuse glioma.Methods 65 cases of diffuse glioma confirmed by histopathology were analyzed for enhancement features on preoperative MRI contrast-enhanced(CE)-T1 WI and CE-T2 FLAIR.The difference in the proportion of enhanced T2 FLAIR-T1WI mismatch between IDH mutation(27 cases)and wild-type(38 cases)diffuse gliomas was compared.Results There were statistically signifi-cant differences(P<0.05)in the enhancement quality and proportion enhancing on CE-T1WI and CE-T2 FLAIR between the IDH mutation and wild-type groups.The difference in the proportion of enhanced T2 FLAIR-T1 WI mismatch was statisti-cally significant(P<0.05)between the two groups.For predicting IDH mutation status,the sensitivity,specificity,and AUC value of the proportion of enhanced mismatch were 81.6%,62.0%,and 0.767,respectively.Conclusion En-hanced T2 FLAIR-T1WI mismatch feature can help predict IDH mutation status in diffuse glioma preoperatively.
