Objective To explore the correlation among kidney function,brain functional connectivity,and cognitive function in healthy middle aged and elderly people observed with multimodal MRI.Methods Resting-state high-resolution MRI scans with standardized neuropsychological assessment were acquired from 49 healthy middle aged and elderly volun-teers with normal cognitive function.Functional connectivity(FC)analysis was performed based on voxel levels.Peripheral venous blood was collected and serum kidney function indices were measured.Correlation and mediation analysis were used to observe the relationship among kidney function,brain FC and cognitive function.Results The FC values between the left hippocampus and the right precentral gyrus of healthy older adults was negatively correlated with the concentration of u-rea;The FC values between the left hippocampusand left middle frontal gyrus,orbital part,right precuneus,right inferior frontal gyrus,orbital part,right inferior frontal gyrus,opercular part,left middle frontal gyrus,and right inferior parietal gy-rus,between the right parahippocampal gyrus and left middle frontal gyrus,bilateral inferior frontal gyrus,orbital part,right superior occipital gyrus,calcarine fissure and surrounding cortex,right inferior parietal,but supramarginal and angular gyri,left precuneus,left precentral gyrus,bilateral superior temporal gyrus were negatively correlated with uric acid(r=-0.546-0.724,all P<0.05).The FC value between the left hippocampus and middle frontal gyrus,orbital part was negatively correlated with verbal fluency test(VFT)(r=-0.388,P=0.008).Further mediation analysis showed that the FC be-tween the left hippocampus and middle frontal gyrus,orbital part partially mediated the effect of uric acid on VFT with a me-diating effect value of 0.0265[95%CI(0.0062,0.0581)].Conclusion Kidney function,brain functional connectivity and cognitive function have certain correlations in healthy middle aged and elderly people.
