血管型白塞病的影像特征分析

摘要
目的 分析血管型白塞病(BD)的临床及影像特征,提高对该疾病的认识.方法 回顾性搜集2020年12月至2023年10月血管型BD患者的临床资料,分析其影像特征.结果 血管型BD患者男女比例4∶1,平均年龄40.3岁.95％患者有口腔溃疡,80％同时存在口腔溃疡及生殖器溃疡,25％皮肤病变,25％眼炎.有大约一半的患者炎症指标升高.所有患者均有血管受累,主要表现为动脉瘤或瘤样扩张、动脉狭窄、静脉血栓.20例患者均应用糖皮质激素或免疫抑制剂治疗.经随访,13例病情稳定,1例无好转,2例死亡,4例失访.结论 血管型BD好发于男性,临床表现多样,动静脉均可受累,影像学检查可以较好地评估动脉瘤或血栓形成,为后续治疗提供依据.
Abstract
Objective To analyze the clinical features and imaging features of Vascular Behçet's Disease(BD)and to improve the recognition of this disease.Methods The clinical manifestations and imaging features of patients with vascu-lar BD from December,2020 to October,2023 were collected.Results Among the 20 cases of BD,The male to female ratio of vascular type BD patients was 4∶1,with an average age of 40.3 years.95％of patients had oral ulcers,80％had both oral and genital ulcers,25％had skin lesions,and 25％had ophthalmitis.About half of the patients had elevated in-flammatory markers.All patients had vascular involvement,mainly manifested as aneurysm or tumor like dilation,arterial stenosis,and venous thrombosis.All 20 patients were treated with glucocorticoids or immunosuppressants.After follow-up,13 cases showed stable conditions,1 case showed no improvement,and 2 cases died.Conclusion Vascular BD is com-mon in males and has diverse clinical manifestations.Both arteries and veins are affected.Imaging examination can effec-tively evaluate aneurysms or venous thrombosis,providing a basis for subsequent treatment.
