Voxel-Mirrored Homotopic Connectivity in Patients with Diabetic Retinopathy:A Resting-State Functional Magnetic Resonance Imaging Study

目的 基于体素镜像同伦连接(voxel-mirrored homotopic connectivity,VMHC)方法,探究糖尿病视网膜病变(diabetic retinopathy,DR)患者大脑半球间功能连接的变化.方法 选取DR患者26例,糖尿病不伴视网膜病变(diabetes mellitus without retinopathy,NDR)患者 30 例行静息态功能磁共振成像(resting-state functional magnetic resonance imaging,rs-fMRI)扫描,比较两组间VMHC值的差异,并分析其与临床指标及神经心理量表评分的相关性.结果 以年龄、性别及教育年限作为协变量,与NDR组相比,DR组双侧舌回、颞上回、丘脑、楔叶及中央后回VMHC值显著降低(FWE校正,体素水平P<0.001,团块水平P<0.05).偏相关分析显示,DR组双侧舌回、楔叶及中央后回的 VMHC 值与病程显著负相关(r=-0.500,P=0.015;r=-0.474,P=0.022;r=-0.485,P=0.019).结论 DR患者存在双侧视觉皮层、听觉皮层、躯体感觉皮层及信息整合中枢协同性受损,且与病程呈负相关,这可能是DR患者脑功能损伤的神经病理机制,能够为临床早期诊断和干预提供有效的影像学途径.
Objective To explore the changes of interhemispheric functional connectivity in patients with diabetic reti-nopathy(DR)by voxel-mirrored homotopic connectivity(VMHC)method.Methods Twenty-six patients with DR and thirty patients with diabetes mellitus without retinopathy(NDR)were selected to undergo resting-state functional magnetic resonance imaging(rs-fMRI)scan.The differences of VMHC values between the two groups were compared,and the corre-lation analysis was performed to assess the relationships between VMHC values and clinical indicators as well as neuropsy-chological scale scores.Results With age,sex and years of education as covariates,DR group showed significantly lower VMHC values in the bilateral lingual gyrus,superior temporal gyrus,thalamus,cuneus and postcentral gyrus compared with NDR group(FWE-corrected,voxel-level P<0.001,cluster-level P<0.05).The partial correlation analysis showed that the VMHC values of bilateral lingual gyrus,cuneus and postcentral gyrus in DR group were negatively correlated with disease duration(r=-0.500,P=0.015;r=-0.474,P=0.022;r=-0.485,P=0.019).Conclusion There is an abnor-mal coordination in the bilateral visual cortex,auditory cortex,somatosensory cortex and information integration center of DR patients,which is negatively correlated with disease duration.This may be the neuropathological mechanism of brain function impairment in DR patients,and it can provide an effective imaging approach for early clinical diagnosis and intervention.
