Objective To observe MRI manifestations of uterine carcinosarcomas(UCS).Methods A retrospective analysis included 23 patients with confirmed UCS by surgical pathology.Twenty patients underwent pelvic enhanced MRI scanning, while 3 patients underwent pelvic plain MRI scanning.MRI manifestations of UCS were analyzed. Results MRI findings in all patients showed uterine cavity dilation and the masses in the uterine cavity.MRI findings include 5 cases of pedunculated exophytic type,16 cases of broad-based exophytic type, and 2 cases of infiltrative growth.T1WI showed low signal in 11 cases and iso-intense signal in 12 cases, with 10 cases showing high signal on T1 WI.T2WI shows uniformly slightly high signal intensity, and in 7 cases, there were flow voids within the lesions on T2WI.DWI showed high signal in-tensity in all cases.ADC value of the solid component was(0.95±0.16)x 10-3mm2/s.Contrast-enhanced scans revealed mild enhancement in 6 cases, moderate enhancement in 10 cases, and significant enhancement in 4 cases. Conclusion UCS on MRI can present as a large intracavitary mass with heterogeneous signal intensity. It may sometimes be accompanied by flow voids, and the degree of enhancement is comparable to or higher than that of the uterine myometrium.
