摘要
目的 分析肾衰竭血透患者社交回避与苦恼水平及影响因素.方法 选取2023 年5 月-2024 年3 月我院收治的定期血透治疗的肾衰竭患者96 例为研究对象,采用社交回避及苦恼量表(SADS)评估肾衰竭血透患者社交回避及苦恼水平,采用一般资料调查问卷、心理弹性量表(CD-RISC)、社会影响量表(SIS)、社会支持评定量表(SSRS)进行调查,采用多元线性回归分析肾衰竭血透患者社交回避及苦恼水平的影响因素.结果 96 例肾衰竭血透患者 SADS评分为(15.42±3.25)分;不同文化程度、血透时间、心理弹性、病耻感、社会支持的肾衰竭血透患者SADS评分比较,差异有统计学意义(P<0.05);多元线性回归分析结果显示,文化程度低、血透时间长、心理弹性低、病耻感高、社会支持低是肾衰竭血透患者社交回避及苦恼水平的影响因素(P<0.05).结论 肾衰竭血透患者社交回避及苦恼水平处于中等,且受多种因素影响,临床应积极开展心理疏导、血透知识宣教等针对性护理措施,帮助患者降低社交回避及苦恼水平.
Abstract
Objective To analyze the levels of social avoidance and distress in renal failure hemodialysis patients and their influencing factors.Method Ninety six renal failure pa-tients admitted to the hospital for regular hemodialysis treatment from May 2023 to March 2024 were selected as the research sub-jects.After completing hemodialysis treatment,the Social Avoidance and Distress Scale(SADS)was used to evaluate the current social avoidance and distress status.A baseline data survey questionnaire,Psychological Resilience Scale(CD-RISC),Social Im-pact Scale(SIS),and Social Support Rating Scale(SSRS)were used to investigate the SADS scores of hemodialysis patients with different characteristics of renal failure,and relevant influencing factors were identified.The SADS score of 96 renal failure hemodi-alysis patients was(15.42±3.25)points;Comparison of SADS scores among renal failure hemodialysis patients with different ed-ucational levels,hemodialysis time,psychological resilience,disease shame,and social support(P<0.05);The results of multi-ple linear regression analysis showed that low educational level,long hemodialysis time,low psychological resilience,high sense of shame,and low social support were the factors affecting the level of social avoidance and distress in renal failure hemodialysis pa-tients(P<0.05).Conclusion The level of social avoidance and distress in renal failure hemodialysis patients is moderate,while low educational level,long hemodialysis time,low psychological resilience,high sense of shame,and low social support are all fac-tors that lead to an increase in social avoidance and distress levels in patients.In clinical practice,targeted nursing measures such as psychological counseling and hemodialysis knowledge education should be actively carried out to help patients reduce their level of social avoidance and distress.
