Incidence and influencing factors of buccal bone plate fracture in bone splitting technique
Objective:To explore the incidence of buccal bone plate fracture in bone splitting technique,and to ana-lyze its influencing factors.Methods:The fresh porcine ribs were collected to construct the models of bone splitting,and they were implanted with implants(diameter of 4.0 mm,length of 10 mm).According to presence or absence of buccal bone plate fracture,there were fracture group and non-fracture group.The buccal bone plate/cortical bone thickness,bone plate extru-sion,displacement angle of buccal bone plate and deviation angle of implants were compared between the two groups.The in-fluencing factors of buccal bone plate fracture were analyzed by Logistic regression model.Results:There were 45 implants in the experiment,and the incidence of buccal bone plate fracture was 33.33%(15/45).The pine skin ratio of buccal midpoint and thickness of buccal basal cancellous bone in fracture group were lower than those in non-fracture group,while horizontal displacement and deviation angle of buccal bone plate were higher than those in non-fracture group(P<0.05).According to Logistic regression analysis,an independent risk factor of buccal bone plate fracture was horizontal displacement of buccal bone plate(P<0.05).According to Pearson correlation analysis,horizontal displacement of buccal bone plate was positively correlated with pine skin ratio of buccal midpoint,deviation angle of buccal bone plate and deviation angle of implants,nega-tively correlated with thickness of buccal basal cancellous bone(P<0.05),and not significantly correlated with bone plate ex-trusion of buccal ridge crest and lingual ridge crest(P>0.05).Conclusion:The horizontal displacement of buccal bone plate is an independent risk factor of buccal bone plate fracture in bone splitting technique.Preoperative evaluation of bone condi-tion in implantation area is beneficial to predict the risk of buccal bone plate fracture.
