Osteocorticotomy with orthodontic traction in patients with periodontitis causing misaligned anterior teeth
Objective:To analyze the therapeutic effect of cortical incision combined with orthodontic treatment in patients with anterior teeth dislocation caused by periodontitis. Methods:A total of 80 patients with anterior teeth caused by periodontitis in our hospital from January 2020 to March 2022 were selected and divided into two groups by random number table method. The control group received orthodontic treatment, and the observation group received corticotomy combined with orthodontic treatment. The clinical effect and the time of orthodontic treatment were observed in the two groups, alveolar bone resorption and bone metabolism indexes[osteocalcin(OC),osteoclast differentiation factor(RANKL),osteopontin(OPG)],serum prostaglandin E2(PGE2),interleukin-1β(IL-1β)were observed before and after treatment. Results:There was no sig-nificant difference between the total effective rate of observation group(100.00%)and control group(92.50%)(P>0.05).The time of fine adjustment, alignment and gap closing in observation group were shorter than those in control group(P<0.05).At 6 months after treatment, the alveolar bone resorption degree of observation group was lower than that of control group,OC and OPG levels were higher than those of control group, and RANKL levels were lower than those of control group(P<0.05).At 3 months after treatment, serum levels of PGE2 and IL-1β in the observation group were lower than those in the control group(P<0.05).Conclusion:Corticotomy can accelerate the orthodontic movement of anterior teeth caused by periodontitis, shorten the orthodontic time, regulate bone metabolism, reduce inflammatory response, and reduce alveolar bone resorption. It is a safe and effective method to accelerate tooth movement in auxiliary orthodontic treatment.
