Objective:Kashgar region in Xinjiang is a high-incidence area for tuberculosis,but testicular/epi-didymal tuberculosis is relatively uncommon.This study aims to summarize the diagnostic and treatment experi-ence of testicular/epididymal tuberculosis in the First People's Hospital of Kashgar over the past 10 years to im-prove the management of this disease.Methods:A retrospective analysis was conducted on the clinical manifesta-tions,imaging findings,treatment methods,pathological features,and outcomes of patients diagnosed with tes-ticular/epididymal tuberculosis at the First People's Hospital of Kashgar from January 2013 to December 2022.Results:A total of 61 patients with testicular/epididymal tuberculosis were collected.The youngest age was 20 years old,the oldest age was 83 years old,and the average age was about(46.22±15.88)years old.Among the 61 patients,32 cases(52.5%)were associated with testicular tuberculosis,14 cases(23%)with simple epididymal tuberculosis,and 15 cases(24.5%)with simple testicular tuberculosis.Forty-two cases had painful symptoms of scrotum,16 cases showed painless mass.In ultrasound examination,there was significant difference between the gray pattern classification of epididymal tuberculosis and testicular tuberculosis(P<0.001).Compared with<50 years old patients,the diffuse type of epididymal tuberculosis was predominant in ≥50 years old patients(P=0.018),the incidence of scrotal sinuses decreased(P=0.043),the abnormal incidence of leukocytes(P=0.017)and neutrophils(P=0.020)increased,and the rate of surgical treatment increased(P=0.014).Of the 61 pa-tients,36 were treated with drugs alone and 25 were treated with drugs combined with surgery.There were 42 follow-up results,of which 5 cases were cured,22 cases were under drug treatment,14 cases recurred after treat-ment,and 1 case had poor curative effect.Conclusion:Testicular/epididymal tuberculosis in our hospital is more common in young and middle-aged people,and scrotal mass and scrotal pain are the most common presenting symptoms.There are significant differences in the ultrasound patterns of testicular and epididymal tuberculosis.Diffuse enlarging heterogeneous lesions are most common in the epididymis,and miliary lesions are most common in the testicles.There are certain differences in clinical imaging manifestations,laboratory tests,and treatment options among patients of different ages.
