Analysis of clinical features and prognostic factors of invasive bladder urothelial carcinoma with carcinoma in situ
Objective:To thoroughly explore the clinical characteristics and prognosis-related factors of inva-sive uroepithelial carcinoma of the bladder with carcinoma in situ,so as to provide an accurate basis for clinical treatment.Methods:The relevant clinical data of patients with invasive uroepithelial carcinoma of the bladder with carcinoma in situ admitted to the First Affiliated Hospital of Zhengzhou University from June 2012 to September 2022 were retrospectively analyzed,including gender,age,whether hematuria,whether lumbar and abdominal pain,whether difficulty in urination and treatment modalities were statistically analyzed.Survival analysis was used to explore the prognosis-related factors,including age,gender,pathologic type,and treatment modality.Re-sults:A total of 156 patients were included,including 89 males and 67 females,with a male-to-female ratio of 1.33:1,aged 23-87 years,with a median age of 62 years.The treatment modalities were mainly radical cystec-tomy or combined adjuvant therapy after radical cystectomy for bladder cancer.The median follow-up time was 33(2-105)months,and the 1-,3-,and 5-year survival rates were 87.1%,72.5%,and 53.6%,respectively.Uni-variate analysis showed hematuria(P=0.004),elevated urinary leukocytes(P=0.035),tumor<2 cm from the ureteral opening(P=0.032),surgical modality(P=0.002),postoperative chemotherapy(P=0.012),patholog-ical staging(P=0.068),and pathological grading(P=0.048)were the factors affecting the survival time of pa-tients with invasive uroepithelial carcinoma of the bladder associated with carcinoma in situ.Multifactorial analysis by Cox showed hematuria(P=0.016),surgical approach(P=0.021),postoperative chemotherapy(P=0.030)and pathological staging(P=0.020)were independent factors affecting the prognosis of patients with invasive uroepithelial carcinoma of the bladder with carcinoma in situ.Conclusion:Invasive urothelial carcinoma of the bladder with carcinoma in situ is rare in clinical practice,and the treatment is complex.Its prognosis is related to factors such as hematuria,surgical method,postoperative chemotherapy,and pathological staging.Early physical examination should be emphasized,and standardized and individualized treatment should be strengthened to reduce the risk of postoperative recurrence.
