小肠镜联合探头式共聚焦激光显微内镜对小肠疾病的观察研究

摘要
目的 探索小肠疾病在探头式共聚焦激光显微内镜(pCLE)图像中的特有表现,以揭示其在小肠疾病实时诊断中的潜在应用价值.方法 纳入2023年2月~2023年6月我院接受小肠镜和pCLE检查的小肠疾病患者48例.结合组织病理学图片特征,分析小肠疾病在pCLE图像中的特殊征象.结果 回顾性分析采集的pCLE图像,可清晰观察到与小肠疾病病理诊断相符的一系列特殊征象,包括但不限于上皮细胞脱落、杯状细胞增多、腺体间距增宽、上皮连续性中断或缺失、腺体结构破坏、隐窝结构异常、荧光素钠渗漏、血管增粗迂曲等.结论 pCLE技术在小肠疾病的诊断中与病理特征具有一致性,其图像中观察到的特有表现可为小肠疾病的实时诊断提供依据,有一定的应用潜力.
Abstract
Objective To explore the unique manifestations of small bowel diseases in probe-based confocal laser endomicroscopy(pCLE)images,with the aim of revealing its potential application value in the real-time diagnosis of small bowel diseases.Methods Forty-eight small bowel diseases patients who underwent enteroscopy and pCLE in our hospital from February,2023 to June,2023 were included.Combined with the feature of histopathology images,analyzed special signs of small bowel disease in pCLE image.Results Retrospective analysis of the collected pCLE images showed that a series of special signs consistent with the pathological diagnosis of small bowel diseases could be clearly observed,including but not limited to epithelial cell shedding,increased goblet cells,widened gland spacing,discontinuity or loss of epithelial integrity,destruction of glandular structure,abnormal crypt structure,leakage of fluorescein sodium,and thickening and tortuosity of blood vessels.Conclusion pCLE technology has the potential to be applied in the diagnosis of small bowel diseases with the consistent pathological characteristics,can provide the basis for the real-time diagnosis of small bowel diseases.
