中性粒细胞联合NLR对缺血性脑卒中取栓术后患者预后的影响

摘要
目的 探讨中性粒细胞及中性粒细胞与淋巴细胞比值(NLR)对缺血性脑卒中取栓术后患者预后的影响.方法 选取2019年1月至2022年1月在蚌埠市第三人民医院卒中病房行取栓术的182例缺血性脑卒中患者作为研究对象,根据预后结果将预后良好的患者作为研究组(110例),预后不良的患者作为对照组(72例).比较两组患者在治疗前、治疗后1 d、治疗后5 d的空腹血糖水平、中性粒细胞数量以及NLR水平;通过logistic多元回归分析探讨中性粒细胞及NLR与患者预后效果间的关系;通过绘制ROC曲线分析中性粒细胞及NLR对患者预后效果的预测效能.结果 在接受治疗后的第1天、第5天,研究组的中性粒细胞及NLR均显著低于对照组,差异有统计学意义(P＜0.05);将因变量美国国立卫生研究院卒中量表(NIHSS)评分设置为分类变量,将预后良好设置为0,预后不良设置为1,在治疗后第5天,基于logistic回归方程使用逐步后退法分析,发现中性粒细胞及NLR与患者的预后效果存在显著关联(P＜0.05);在接受治疗后的第5天,AUC数值由大到小分别为两项联合、NLR及中性粒细胞,其中,两项联合的AUC数值为0.893.结论 中性粒细胞及NLR均是评估缺血性脑卒中取栓术后患者预后结果的有效指标,二者联合预测价值更高.
Abstract
Objective To analyze the effect of neutrophil,neutrophil to lymphocyte ratio(NLR)on the prognosis of patients with ischemic stroke after thrombectomy.Methods A total of 182 patients with ischemic stroke who underwent thrombectomy were selected as research subjects from the stroke ward of the Third People's Hospital of Bengbu from January 2019 to January 2022.According to the prognosis results,the patients with good prognosis were assigned as research group(110 cases)and patients with poor prognosis as control group(72 cases).Fasting blood glucose level,neutrophil count and NLR level were compared between two groups of patients before treatment,1 and 5 days after treatment.Logistic multiple regression analysis was applied to explore the association of neutrophils and NLR with the prognosis of the patients.ROC curve analysis was performed to in-vestigate the predictive efficacy of neutrophils and NLR on the prognosis of the patients.Results On the 1st and 5th day after treatment,the neutrophils and NLR in research group were significantly lower than those in control group(P＜0.05).The dependent variable NIHSS score was set as a categorical variable,the good prognosis as 0 and the poor prognosis as 1.Logistic regression equation-based step-wise regression analysis revealed that neutrophils and NLR were significantly associated with the out-come of the patient's prognosis on the 5th day after treatment(P＜0.05).On the 5th day after treat-ment,AUC values in descending order was the combination of neutrophils and NLR,NLR and neutro-phils,among which,the AUC of the combination of neutrophils and NLR was 0.893.Conclusion Neutrophils and NLR are both effective indicators in evaluating the prognosis of patients with ischemic stroke after thrombectomy,and their combined predictive value is high.
