颅脑MRI和肛门括约肌肌电图联合α-突触核蛋白在帕金森病和多系统萎缩P型患者中的应用

摘要
目的 探讨颅脑MRI、肛门括约肌肌电图联合唾液α-突触核蛋白(α-syn)水平在帕金森病(PD)和多系统萎缩P型(MSA-P)患者中的鉴别诊断价值.方法 选取2016年9月至2023年6月福建省老年医院收治的142例患者,其中81例PD患者作为PD组、41例MSA-P患者作为MSA-P组,比较两组患者在颅脑MRI壳核裂隙征、肛门括约肌肌电图的卫星电位及唾液α-syn水平上的差异.结果 MSA-P组患者壳核裂隙征和卫星电位的出现率显著高于PD组,唾液α-syn水平也明显高于PD组.多元回归和ROC曲线分析显示,壳核裂隙征、唾液的α-syn水平及卫星电位的联合应用可鉴别PD和MSA-P患者.结论 联合应用颅脑MRI、肛门括约肌肌电图及唾液α-syn可以有效地鉴别PD和MSA-P型,为临床诊断提供重要的辅助手段.
Abstract
Objective To explore the diagnostic values of cranial MRI,anal sphincter electro-myography(AS-EMG)combined salivary α-synuclein(α-syn)level in patients with Parkinsons disease(PD)and patients with multiple system atrophy-parkinsonian type(MSA-P).Methods A total of 142 patients admitted to Fujian Provincial Geriatric Hospital from September 2016 to June 2023 were selected,among which,81 patients with PD were selected as PD group,and 41 with MSA-P patients as MSA-P group.Putaminal slit in Cranial MRI,satellite potential in AS-EMG and salivary α-syn lev-el were compared between two groups.Results The incidences of putaminal slit and satellite potential in MSA-P group were significantly higher than those in PD group,and α-syn level was also significant-ly higher than that of PD group.Multiple regression and ROC curve analyses showed that the com-bined application of putaminal slit,salivary α-syn and satellite potential may differentiate the patients with PD and the patients with MSA-P.Conclusion Combined application of cranial MRI,AS-EMG and salivary α-syn level can effectively distinguish between PD and MSA-P types,providing important auxiliary means for clinical diagnosis.
