摘要

目的 探讨中性粒细胞-淋巴细胞比值(NLR)、单核细胞-淋巴细胞比值(MLR)、血小板-淋巴细胞比值(PLR)与抗N-甲基-D-天冬氨酸受体(NMDAR)脑炎的关系.方法 选择2014年6月至2021年6月在河北医科大学第二医院神经内科确诊为抗NMDAR脑炎的患者42例作为抗NMDAR脑炎组,同时选取性别、年龄与抗NMDAR脑炎组匹配的健康人群作为健康对照组,收集患者的临床资料,计算NLR、MLR及PLR,根据疾病高峰时改良Rankin量表(mRS)评分评估抗NMDAR脑炎患者病情,并将其分为轻度组和重度组,将其与健康对照组42例进行比较,并比较轻度组和重度组患者的年龄、外周血细胞计数、NLR、MLR及PLR的差异.结果 抗NMDAR脑炎组患者的白细胞、中性粒细胞、单核细胞、NLR、MLR及PLR较健康对照组比较显著升高(P<0.05),血小板、淋巴细胞较健康对照组显著降低(P<0.05).重度抗NMDAR脑炎组患者的白细胞、中性粒细胞、NLR及PLR水平显著高于轻度抗NMDAR脑炎组(P<0.05),淋巴细胞显著低于轻度抗NMDAR脑炎组(P<0.05).NLR水平升高与重度抗NMDAR脑炎有显著相关性(P<0.05).结论 NLR是重度抗NMDAR脑炎的独立危险因素.

Abstract

Objective To explore the association of neutrophil-to-lymphocyte ratio(NLR),monocyte-to-lymphocyte ratio(MLR)and platelet-to-lymphocyte ratio(PLR)with anti-N-methyl-D-aspartate receptor(NMDAR)encephalitis.Methods A total of 42 patients diagnosed as anti-NMDAR encephalitis were selected as anti-NMDAR encephalitis group at the Department of Neurology,the Second Hospital of Hebei Medical University from June 2014 to June 2021.Healthy individuals who matched anti-NMDAR encephalitis group in terms of gender,age were simultaneously select as healthy control group.Clinical data of the patients were collected.NLR,MLR and PLR were calculated.Modified Rankin Scale(mRS)during the peak of the disease was applied to assess disease condition of patients with anti NMDAR encephalitis,and the patients were divided into mild disease and severe dis-ease groups,which were compared with healthy control group.The differences in age,peripheral blood cell count,NLR,MLR and PLR were compared between mild disease and severe disease groups.Results Leukocytes,neutrophils,monocytes,NLR,MLR and PLR were significantly high-er in anti-NMDAR group than those in healthy control group(P<0.05),while platelets and lympho-cytes were significantly lower than those in healthy control group(P<0.05).Leukocytes,neutrophils and the levels of NLR and PLR were significantly higher in the patients of severe anti-NMDAR en-cephalitis group than those in the patients of mild anti-NMDAR encephalitis group(P<0.05),while lymphocytes were significantly lower than those in mild anti-NMDAR encephalitis group(P<0.05).There was a significant correlation between the elevated NLR levels and severe anti-NMDAR encepha-litis.Conclusion NLR is an independent risk factor for severe anti-NMDAR encephalitis.
