摘要

目的 探讨简化正中神经寸移对常规肌电图检查正常的腕管综合征(CTS)的诊断价值.方法 以正常受试者为检测对象先测出正中神经(运动和感觉)寸移2 cm间隔平均传导时间,再以最长时间间隔计算出95%区间,以上限为判断间隔时间是否异常的标准.对临床考虑为CTS的患者进行常规肌电图检测,筛选出肌电图正常的患者,再进行简化正中神经寸移检测.结果 正常人正中神经腕段运动寸移从腕的近段到远端3个节段的传导时间分别是(0.32±0.21)ms、(0.59±0.19)ms和(0.41±0.23)ms,以最长时间第2段为对象确立2 cm距离神经传导正常值上限为1 ms,以超过1 ms为异常;同样正中神经腕段逆向感觉神经传导寸移从腕的近段到远端3个节段的传导时间分别是(0.21±0.21)ms、(0.39±0.19)ms和(0.54±0.23)ms,以最长时间第3段为对象,95%可信区间确定正中神经寸移2 cm距离传导时间上限为1 ms,超过1 ms为异常.简化正中神经寸移检测本组5例不典型症状肌电图正常的CTS患者均存在不同程度的传导阻滞.结论 简化正中神经寸移有助于对常规肌电图检测正常的CTS的诊断.

Abstract

Objective To investigate the value of median nerve inching in carpal tunnel syndrome(CTS)with normal conduction time.Method The average conduction time of the median nerve(motor and sensory)at a 2 cm interval was measured in normal subjects.95%interval was calculated based on the lon-gest time interval.The upper limit was used as the standard to determine whether the interval time was ab-normal.Routine electromyography testing was performed on patients clinically considered to have CTS to i-dentify the patients with normal electromyography.Identified patients were examined by simplified median nerve inching testing.Results The conduction times of the wrist segment movement of the median nerve in normal individuals from the proximal to three distal segments of the wrist were(0.32±0.21)ms,(0.51±0.19)ms and(0.41±0.23)ms,respectively.The second segment was the longest and selected as the sub-ject for defining the upper limit of the normal value of nerve conduction at a distance of 2 cm was 1 ms.Any exceeding 1 ms was considered as abnormal.The conduction times of the reverse sensory nerve conduction of the median nerve wrist segment from the proximal to three distal segments were(0.21±0.21)ms,(0.39±0.19)ms,and(0.54±0.23)ms,respectively.The third segment was the longest and selected as the sub-ject for 95%confidence interval-determined the upper limit of nerve conduction at a distance of 2 cm was 1 ms.Any exceeding 1 ms was considered as abnormal.Simplified median nerve inching revealed that all 5 CTS patients with atypical symptoms and normal electromyography in this group had varying degrees of con-duction blockade.Conclusion Simplifying the median nerve inching is helpful for the diagnosis of CTS with normal electromyography.
