摘要
目的 探讨癫痫患儿的生存质量现状及影响因素.方法 回顾性分析郑州大学第一附属医院儿科2023年1～12月收治的68例癫痫患儿(癫痫组)与30名健康儿童(正常对照组)的资料,采用儿童生存质量量表(PedsQL 4.0)对两组进行评估,了解其生存质量现状.收集癫痫组患儿的临床资料,包括疾病因素、家庭因素、免疫接种、医疗保障、教育情况及社会支持等,分析患儿生存质量的影响因素.结果 癫痫组患儿PedsQL 4.0各项目评分与总分均低于正常对照组,差异有统计学意义(P＜0.05).不同性别、年龄、起病年龄、病程、距上次发作时间、口服药物种类、发育情况、父母焦虑情况、患病对家庭经济的影响、家庭教养方式、家庭情感支持、居住地、免疫接种及教育情况的PedsQL 4.0评分均存在明显差异(P＜0.05).多因素二元logistic回归分析显示发育异常、患病对家庭经济的影响、口服药物种类≥2种、距上次发作时间≤2年、病程≥1年是癫痫患儿生存质量的危险因素(P＜0.05).结论 癫痫患儿的生存质量偏低,其影响因素包括发育异常、患病对家庭经济的影响、口服药物种类≥2种、距上次发作时间≤2年、病程≥1年.
Abstract
Objective To explore the current status of quality of life in children with epilepsy and in-fluencing factors.Methods A retrospective analysis was conducted on the data of 68 children with epilepsy(epilepsy group)and 30 healthy children(normal control group)admitted to the Department of Pediatrics,the First Affiliated Hospital of Zhengzhou University from January to December 2023.Pediatric Quality of Life Inventory(PedsQL 4.0)was used to evaluate both groups and understand their current status of the quality of life.Clinical data of children with epilepsy were collected,including disease factors,family factors,immunization,medical security,education status and social support.The factors affecting the quality of life of these children were analyzed.Results Each item score in PedsQL 4.0 and total scores were lower in epi-lepsy group than those in normal control group(P＜0.05).There were significant differences in PedsQL 4.0 scores among different genders,ages,onset ages,disease duration,time since last episode,types of oral medications,developmental status,parental anxiety,impact of illness on family economy,parenting styles,family emotional support,place of residence,immunization and education status(P＜0.05).Multivariate bi-nary logistic regression analysis showed that developmental abnormalities,the impact of illness on family e-conomy,oral medication types 2,time to last seizure≤2 years and disease duration 1 year were risk factors for the quality of life of the children with epilepsy(P＜0.05).Conclusion The quality of life of chil-dren with epilepsy is relatively low,and its influencing factors include developmental abnormalities,the im-pact of illness on family economy,2 types of oral medications,≤2 years since the last seizure and 1 year of disease course.
