Objective To compare the differences in clinical pathological feature, prognosis and immune characteristics between elderly and young patients by analyzing the clinical and pathological features of young and elderly glioblastoma(GBM)patients. Methods The clinical data of 447 GBM patients who underwent surgical treatment at Beijing Tiantan Hospital from January 2022 to June 2024 were analyzed retrospectively.Prognostic and immunological of RNA-seq data from young and elderly GBM patients were analyzed by using CGGA and TCGA databases. Results For the part of clinical characteristics, young GBM patients and elderly GBM patients showed differences in gender,MGMT methylation,TERT mutation,EGFR amplification, and TP53 mutation, while there were no significant differences in tumor location, side, size and immunohistochemistry of Ki-67.The overall survival(OS)of young GBM patients was significantly longer than that of elderly GBM patients. In terms of immunity, elderly patients showed more obvious characteristics of immunosuppressive microenvironment. Conclusions Young GBM patients have longer survival time and better prognosis than elderly GBM patients, and there are significant differences in clinicopathological features and immune microenvironment between young and elderly GBM patients.
