Objective To compare the remission rate, recurrence rate and complication rate of transsphenoidal surgery(TSS)and radiotherapy(RT)in the treatment of recurrent Cushing's disease(CD),aiming to provide clinical evidence support for patients'diagnosis and treatment options. Methods The retrospective study was conducted on 48 patients with recurrent CD admitted to Department of Neurosurgery, the Chinese PLA General Hospital from January 2010 to December 2022.The patients were divided into the TSS group(26 patients)and the RT group(22 patients)based on different treatment methods. The remission rate, recurrence rate and complication rate before and after treatment were analyzed for the two groups at one year after surgery. Results All patients showed varying extent of improvement after treatment(P<0.05).The remission rates of the two groups were 57.7%(37.3%-78.0%)and 50.0%(27.3%-72.7%),with no significant difference(P=0.594).The recurrence rates of the two groups after 1 year were 7.7%(3.3%-18.7%)and 9.1%(4.0%-22.1%),respectively,with no significant difference(P=0.861).The complication rates of the two groups were 7.7%(-3.3%-18.7%)and 4.5%(-4.9%-14.0%),respectively,with no significant difference(P=0.654).Conclusions This study compares two intervention measures(surgery and RT)currently used in clinical practice for recurrent CD.It shows that there are no significant differences in remission rate, recurrence rate, and complication rate between TSS and RT.
