目的 探讨结直肠癌组织微小RNA(miR)-383、miR-4458、miR-93表达水平及临床意义.方法 2018年10月~2020年10月手术治疗的124例结直肠癌术中切除的结直肠癌组织标本作为结直肠癌组,其癌旁正常组织作为对照组.采用实时荧光定量-聚合酶链式反应(qRT-PCR)法检测组织 miR-383、miR-4458、miR-93 表达水平;结直肠癌组织 miR-383、miR-4458、miR-93表达与病人预后的关系采用Kaplan-Meier曲线分析;Cox回归分析结直肠癌病人预后的影响因素.结果 与对照组比较,结直肠癌组miR-383、miR-4458、miR-93表达水平较低(P<0.05).结直肠癌组织miR-383、miR-4458、miR-93表达与肿瘤直径、TNM分期、淋巴结转移、浸润深度等临床病理特征有关(P<0.05).结直肠癌组织miR-383高表达病人3年生存率(55/63,87.30%)高于miR-383 低表达病人(41/61,67.21%)(x2=7.650,P<0.05);结直肠癌组织miR-4458高表达病人3年生存率(53/60,88.33%)高于 miR-4458 低表达病人(43/64,67.18%)(x2=8.566,P<0.05);结直肠癌组织miR-93高表达病人3年生存率(51/59,86.44%)高于miR-93低表达病人(45/65,69.23%)(x2=5.612,P<0.05).多因素 Cox 回归分析结果显示,miR-383、miR-4458、miR-93 是结直肠癌病人预后的保护因素(P<0.05),肿瘤直径、TNM分期、淋巴结转移、浸润深度是结直肠癌病人预后的危险因素(P<0.05).通过Cox回归模型预测结直肠癌预后的曲线下面积(AUC)为0.917,95%CI 为 0.854~0.959(Z=12.338,P<0.05),敏感度为 94.29%,特异度为 87.64%.结论 结直肠癌组织miR-383、miR-4458、miR-93表达水平显著降低,三者表达水平与结直肠癌病人临床病理特征和预后有关.
Expression levels and clinical significance of miR-383,miR-4458,and miR-93 in colorectal cancer tissue
Objective To investigate the expression levels and clinical significance of microRNA-383(miR-383),microRNA-4458(miR-4458),and microRNA-93(miR-93)in colorectal cancer tissue.Methods From October 2018 to October 2020,124 colorectal cancer patients who underwent surgical treatment in our hospital were regarded as the colorectal cancer group,and their adjacent normal tissues were as the adjacent control group.Real-time fluorescence quantitative polymerase chain reaction(qRT-PCR)method was applied to detect the expression levels of miR-383,miR-4458,and miR-93 in tissues;the relationship between the expression of miR-383,miR-4458,and miR-93 in colorectal cancer tissue and patient prognosis was analyzed using Kaplan-Meier curve;Cox regression was applied to analyze the prognostic factors of colorectal cancer patients.Results Compared with the adjacent control group,the expression levels of miR-383,miR-4458,and miR-93 in the colorectal cancer group were obviously lower(P<0.05).The expression of miR-383,miR-4458,and miR-93 in colorectal cancer tissue was related to clinical and pathological features such as tumor diameter,TNM staging,lymph node metastasis,and depth of invasion(P<0.05).The three-year survival rate of patients with high expression of miR-383 in colorectal cancer tissue(55/63,87.30%)was higher than that of patients with low expression of miR-383(41/61,67.21%)(x2=7.650,P<0.05);the three-year survival rate of patients with high expression of miR-4458 in colorectal cancer tissue(53/60,88.33%)was higher than that of patients with low expression of miR-4458(43/64,67.18%)(x2=8.566,P<0.05);the three-year survival rate of patients with high expression of miR-93 in colorectal cancer tissue(51/59,86.44%)was higher than that of patients with low expression of miR-93(45/65,69.23%)(x2=5.612,P<0.05).According to multivariate Cox regression analysis,miR-383,miR-4458,and miR-93 are protective factors for the prognosis of colorectal cancer patients(P<0.05),while tumor diameter,TNM staging,lymph node metastasis,and depth of infiltration were risk factors for the prognosis of colorectal cancer patients(P<0.05).The area under the curve(AUC)predicted by Cox regression model is 0.917,95%CI is 0.854~0.959(Z=12.338,P<0.05),the sensitivity is 94.29%,and the specificity is 87.64%.Conclusion The expression levels of miR-383,miR-4458,and miR-93 in colorectal cancer tissue are obviously reduced,and their expression levels are related to the clinical pathological characteristics and prognosis of colorectal cancer patients.
