[Objective]To analyze the characteristics and prognostic factors of lower esophageal adeno-carcinoma in the elderly.[Methods]The total of 276 elderly patients with lower esophageal adenocarcinoma were selected as the research object, and the incidence characteristics of lower esophageal adenocarcinoma were analyzed. The elderly patients with lower esophageal adenocarcinoma were followed up to January 2020.The follow-up time was 12-60 months, with an average follow-up time of(42.35±6.17)months.The prognosis of the patients was analyzed.Univariate analysis was used to analyze the related fac-tors affecting the prognosis of lower esophageal adenocarcinoma in the elderly, and logistic regression mod-el was used to analyze the independent risk factors affecting the prognosis of lower esophageal adenocarci-noma in the elderly.[Results]The characteristics of the elderly patients with lower esophageal adenocarci-noma were mainly ulcerative, and the degree of differentiation was mainly poorly differentiated. The elderly patients with lower esophageal adenocarcinoma were followed up.The end point was death or the end of follow-up.According to the prognosis, there were 197 cases in the survival group and 79 cases in the death group.Univariate analysis showed that there was no significant difference in sex, drinking history, smoking history, family history, degree of differentiation,T stage and postoperative adjuvant radiotherapy and chem-otherapy between survival group and death group(P>0.05).There were significant differences in tumor diameter,TNM stage, lymph node metastasis and nerve invasion between survival group and death group(P<0.05).Logistic model analysis showed that tumor diameter,TNM stage, lymph node metastasis and nerve invasion were independent risk factors affecting the prognosis of elderly patients with lower esopha-geal adenocarcinoma.[Conclusion]Ulcerative adenocarcinoma is the main type of lower esophageal adeno-carcinoma in the elderly, and the degree of differentiation is mainly poorly differentiated.Tumor diameter,TNM stage, lymph node metastasis and nerve invasion were independent risk factors for the prognosis of lower esophageal adenocarcinoma in the elderly.
