视听联合分散技术对结肠镜检查患者干预效果的Meta分析

摘要
[目的]通过Meta分析评价视听联合分散技术在结肠镜检查患者中的应用效果.[方法]计算机检索万方、中国知网、Web of Science、Cochrane Library、PubMed、Embase数据库中关于视听联合分散技术对结肠镜检查患者干预效果的随机对照试验,检索时限为建库至2022年3月.由2名研究人员分别对文献进行筛选、质量评价和资料提取,采用RevMan 5.4软件进行Meta分析.[结果]共纳入6篇文献,包括819例结肠镜检查患者.Meta分析结果显示,视听联合分散能显著降低患者在结肠镜检查过程中的疼痛水平(WMD=-1.27,95％CI=-1.64～-0.89,P＜0.00001)、有效缓解患者检查过程中焦虑状态(SMD=-0.34,95％CI=-0.61～-0.08,P=0.01),显著提高患者再次进行结肠镜检查的意愿(OR=2.21,95％CI=1.04～4.69,P=0.04);在结肠镜检查的总时间方面,视听分散组与对照组比较差异无统计学意义(WMD=-0.12,95％CI=-0.78～0.55,P=0.73).[结论]视听联合分散技术能有效降低结肠镜检查患者的疼痛和焦虑水平,提高其再次进行结肠镜检查的意愿,但仍需更多高质量的随机对照试验进一步验证.
Abstract
[Objective]To evaluate the impact of audiovisual distraction technique on patients undergoing colonoscopy through meta-analysis.[Methods]We searched Wanfang,CNKI,Web of Science,Cochrane Li-brary,PubMed,Embase databases from their inception to March 2022,and we included randomized con-trolled trials(RCTs)reporting the effectiveness of audiovisual distraction technique in patients undergoing colonoscopy.Two researchers independently screened the literature,evaluated the quality and extracted the data.Review Manager(RevMan)software(Version 5.4)was used for the statistical analysis.[Results]Six studies involving 819 patients were included in this meta-analysis.The results of meta-analysis showed that audiovisual distraction significantly reduced the degree of pain(WMD=-1.27,95％CI=-1.640.89,P＜0.00001)and anxiety during the procedure(SMD=-0.34,95％CI=-0.610.08,P=0.01),and improved patient's willingness to repeat the procedure(OR=2.21,95％CI=1.044.69,P=0.04).There was no sta-tistically significant difference in total procedure time(WMD=-0.12,95％CI=-0.780.55,P=0.73).[Conclusion]Audiovisual distraction technique can alleviate the degree of pain and anxiety in patients un-dergoing colonoscopy,and enhance their willingness to repeat the procedure.However,the conclusion of this study needs to be confirmed by more high-quality RCTs.
