肠镜不同退镜方法对结肠黑变病息肉检出的对比研究

摘要
[目的]研究电子肠镜常规白光退镜、色素内镜退镜及电子肠镜联合人工智能技术(artificial intelligence,AI)退镜3种方法对结肠黑变病(melanosis coli,MC)息肉检出的影响.[方法]纳入行电子肠镜检查诊断为MC的患者120例,肠镜检查过程中分别对其行不同退镜方法操作,从而进一步研究电子肠镜常规白光退镜、色素内镜退镜及电子肠镜联合AI技术退镜3种方法对MC息肉检出的影响.[结果]电子肠镜常规白光退镜组(A组)中发现7例结肠息肉,息肉检出率为17.50%;色素内镜退镜组(B组)及电子肠镜联合AI技术退镜组(C组)中各发现17例、19例结肠息肉,息肉检出率分别为42.50%、47.50%;B组、C组与A组比较,均差异具有统计学意义(P分别为0.043、0.012).A 组中有 2 例息肉<0.5 cm,4 例 0.5~1.0 cm,1 例>1.0 cm;B 组中有 11 例<0.5 cm,6 例 0.5~1.0 cm,0 例>1.0 cm;C 组中有 13 例<0.5 cm,6 例 0.5 cm-1.0 cm,0 例>1.0 cm.B 组及 C 组对<0.5 cm 的结肠息肉检出数明显高于A组,差异具有统计学意义(P分别为0.037、0.024).[结论]MC患者行电子肠镜检查时,色素内镜或常规电子肠镜联合AI技术退镜观察,对较大结肠息肉的检出无显著意义,但其可以明显提高结肠微小息肉的检出.
Abstract
[Objective]To study the effects of three methods:conventional white light,chromoendosco-py,and colonoscopy combined with artificial intelligence(AI)retraction on the detection of polyps in mela-nosis coli(MC).[Methods]A total of 120 patients with MC were included.In the process of colonoscopy,different methods of retraction were performed on them,so as to further study the influence of three meth-ods of conventional white light,chromoendoscopy,and colonoscopy combined with AI on the detection of MC polyps.[Results]Seven colon polyps were found in the conventional white light group,and the polyp detection rate was 17.50%,while seventeen and nineteen colon polyps were found in the chromoendoscopy group and the colonoscopy combined with AI group respectively.The detection rates of polyps were 42.50%and 47.50%.The difference was statistically significant(P=0.043,0.012).There were 2 cases of polyps<0.5 cm,4 cases of 0.5-1.0 cm,and 1 case of>1.0 cm in the conventional white light group;11 cases were<0.5 cm,6 cases were 0.5-1.0 cm,and 0 cases were>1.0 cm in the chromoendoscopy;and 13 cases were<0.5 cm,6 cases were 0.5-1.0 cm,0 cases>1.0 cm in the colonoscopy combined with AI group.The number of colon polyps<0.5 cm in the chromoendoscopy group and the colonoscopy combined with AI group was significantly higher than that in conventional white light group,and the difference was statistically significant(P=0.037,0.024).[Conclusion]When MC patients undergo colonoscopy,chromoen-doscopy or colonoscopy combined with AI to withdraw from the microscope has no significant effect on the detection of large colon polyps,but it can significantly improve the detection of small colon polyps.
