负载抗真菌药的骨水泥治疗骨与关节真菌感染文献分析

目的 了解负载抗真菌药的骨水泥治疗骨与关节真菌感染的应用现状。方法 检索PubMed、Web of Science、Elsevier Science Direct、Embase、Springer-link、Wiley Online Library、中国知网及万方数据知识服务平台等数据库中关于使用负载抗真菌药的骨水泥治疗骨与关节真菌感染的临床案例,检索时间自数据库建库至2023年1月,提取相关数据进行描述性统计分析。结果 共收集病例16例,男7例,女9例,平均年龄为(64。56±10。33)岁;局部植入负载氟康唑的骨水泥3例,两性霉素B 9例,伏立康唑 5例(其中1例同时植入伏立康唑和两性霉素B);只有8例病例报告记录了使用骨水泥的种类,7例使用Palacos,1例使用Simplex和Synicem 1 g。15例使用二期翻修术,1例使用一期翻修术;总体治愈率为81。25%。结论 使用负载抗真菌药的骨水泥联合全身抗真菌药配合二期翻修术是一种有效治疗骨与关节真菌感染的策略,但最适品种和剂量有待进一步研究。
Literature analysis of antifungal-loaded bone cement in the treatment of fungal infections in bones and joints
Objective To investigate the application of antifungal-loaded bone cement in the treatment of fungal infec-tion of bones and joints.Methods PubMed,Web of Science,Elsevier Science Direct,Embase,Springer-link,Wiley Online Library,CNKI,and Wanfang(up to January 2023)were searched,clinical cases using antifungal-loaded bone cement to treat fungal infections were collected,and relevant data were extracted for descriptive statistical analysis.Results A total of 16 cases were collected,including 8 males and 8 females,with an average age of(64.56±10.33)years.Among them,3 involved flu-conazole,9 involved amphotericin B,and 5 involved voriconazole-loaded bone cement(including 1 case of simultaneous im-plantation of fluconazole and amphotericin B).Only 8 cases reports documented the type of bone cement used,7 cases with Palacos and 1 case with Simplex and Synicem 1 g.A tatal of 15 cases underwent second-stage revision,and 1 case underwent first-stage revision.The overall cure rate was 81.25%.Conclusion The use of antifungal-loaded bone cement combined with systemic antifungal drugs and second-stage revision surgery is an effective strategy for treating fungal infections in bones and joints,but further research is needed to determine the optimal variety and dosage.
