目的 探讨依达拉奉对行胫骨平台骨折切开内固定术患者缺血组织的保护作用。方法 将2022年8月至2023年7月南通市海门区人民医院收治的拟行胫骨平台骨折切开内固定术的患者依据随机数字表法分成依达拉奉组与对照组。麻醉诱导后，依达拉奉组静脉滴注0。5 mg/kg依达拉奉+100 mL 0。9%氯化钠注射液，对照组静脉滴注100 mL 0。9%氯化钠注射液，30 min内滴完。观察两组的血清乳酸、血钾、肌红蛋白、氧化应激指标水平，镇痛效果，康复指标变化及不良事件发生率。结果 共纳入90例患者，两组各45例。两组患者术后1 d的血清乳酸、肌红蛋白较术前均升高，上止血带后1 h(T1)、松止血带后1 h(T2)的丙二醛水平较上止血带前(T0)均升高，但依达拉奉组均低于对照组;T1及T2超氧化物歧化酶水平较T0均降低，但依达拉奉组高于对照组;依达拉奉组的术后疼痛及麻木视觉模拟评分、镇痛药用量、感觉及运动功能恢复时间、术后住院时间、不良事件发生率均低于对照组，差异均有统计学意义(P<0。05)。结论 依达拉奉具有缺血组织与神经保护作用，能减轻机体应激反应及术后疼痛，加快术后恢复，降低不良事件发生率。
The protective effect of edaravone on ischemic tissue in patients undergoing open internal fixation surgery for tibial plateau fractures
Objective To explore the protective effect of edaravone on ischemic tissue in patients undergoing open internal fixation surgery for tibial plateau fractures.Methods Patients scheduled for open internal fixation surgery of tibial plateau fracture at Nantong Haimen District People's Hospital from August 2022 to July 2023 were randomly divided into the edaravone group and the control group.Following anesthesia induction,the edaravone group received an intravenous infusion of 0.5 mg/kg edaravone diluted in 100 mL of 0.9%sodium chloride injection,while the control group received intravenous infusion of 100 mL of 0.9%sodium chloride injection,both administered within 30 minutes.Serum lactate,potassium ions,myoglobin,oxidative stress markers,analgesic effects,rehabilitation indices,and the incidence of adverse events were observed and recorded.Results A total of 90 patients were included,with 45 in each group.The serum lactace and myoglobin of the two groups were increased 1 day after surgery compared with before surgery.The malondialdehyde levels at 1 hour after tourniquet application(T1)and 1 hour after tourniquet release(T2)were both higher than that before tourniquet application(T0),but the levels in the edaravone group were lower than those in the control group.The superoxide dismutase levels at T1 and T2 were both lower than that at T0,but the lev-els in the edaravone group were higher than those in the control group.The visual analog scale scores of postoperative pain and numbness,analgesic consumption,sensory and motor function recovery time,postoperative hospital stay,and incidence of ad-verse events in the edaravone group were all lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Edaravone provides protective effects on ischemic tissue and nerves,alleviates stress reactions and postoperative pain,accelerates postoperative recovery,and lowers the incidence of adverse events.
