A case of pharmaceutical practice in the treatment of infective endocarditis based on vancomycin therapeutic drug monitoring
This paper presents a case of clinical pharmacist's involvement in the treatment of infective endocarditis.After pharmacists adjusted the dosing regimen based on the plasma concentration of vancomycin and recommendations of domestic and foreign guidelines,the plasma concentration of vancomycin reached the target range,and the infection of the patient was well controlled.During the process,the creatinine and urea values were in the normal range.This case highlights the importance of individualized drug regimen adjustments based on therapeutic drug monitoring,which can aid in the timely control of serious infections caused by methicillin-resistant staphylococcus aureus.
