用药复杂患者居家药物治疗管理模式构建与效果评价

摘要
目的 构建用药复杂患者居家药物治疗管理(MTM)模式并评价实施效果.方法 2021-2023年通过北京京煤集团总医院药学门诊、出院患者、社区家医系统和药物治疗风险监测系统不同路径收集用药复杂患者,开展居家MTM;通过优化MTM流程和方法等内容构建管理模式,并评价实施效果.结果 共170例患者完成居家MTM,管理前药物治疗目标均不达标,管理后达标率为91.76%,用药依从性评分达标率从管理前的17.1%提升至管理后的79.41%,管理前后比较差异有统计学意义(P<0.05).患者对自身疾病的认知和管理能力也得到提高.结论 居家MTM模式可提高患者药物治疗的有效性、安全性和依从性.
Abstract
Objective To develop a model for home medication therapy management(MTM)for patients with complex medication use and to evaluate its implementation effectiveness.Methods From 2021 to 2023,patients with complex medication use were collected at Beijing Jingmei Group General Hospital,including the outpatient pharmacy clinic,discharged patients,community-based home healthcare system,and medication risk monitoring system.MTM was then conducted.The management model was constructed by optimizing the MTM processes and methods,and the implementation effect was evaluated.Results A total of 170 patients completed home MTM.Before management,none of the patients met the medication therapy targets.After management,the target achievement rate increased to 91.76%,and the medication adherence score achievement rate improved from 17.1%to 79.41%,there was a statistically significant difference in the comparison before and after management(P<0.05),and patients's understanding and management ability of their own diseases were enhanced.Conclusion The home MTM model can improve the effectiveness,safety and adherence of patients'medication therapy.
