摘要
《中国精神科治疗药物监测临床应用专家共识(2022年版)》(简称《共识》)为精神科治疗药物监测(TDM)的临床应用提供了规范和指导,确立了行业标准.本文探讨了该共识与国外指南部分药物治疗参考浓度范围的差异.核心专家重点关注谷浓度采样和超实验室警戒浓度时的临床决策问题.此外,还就超适应证用药是否需要进行治疗药物监测(TDM)以及TDM的药物经济学优势等问题进行了解读.未来研究方向包括开发自动化结果解释系统、深入基础研究和开展高证据等级的临床研究.通过这些讨论,旨在进一步规范TDM的实施,并为精神科TDM的未来发展提供清晰思路.
Abstract
"Expert consensus on clinical application of psychiatric therapeutic drug monitoring in China(2022 edition)"provides standards and guidance for the clinical application of therapeutic drug monitoring(TDM)in psychiatry,establishing industry benchmarks.This article explores the differences in the therapeutic reference ranges for certain drugs between this consensus and foreign guidelines.Core experts are focusing on two main issues:trough concentration sampling and clinical decision-making when drug concentrations exceed laboratory alert levels.Additionally,it provides interpretations on whether TDM is needed for off-label drug use and the economic advantages of TDM.Future research directions include developing automated result interpretation systems,deepening foundational research,and conducting high-quality clinical studies.The aim is to further standardize the implementation of TDM and provide a clear direction for its future development in psychiatry.
