Comparison of the effects of closed reduction and Kirschner wire fixation under ultrasound guidance versus C-arm fluoroscopy on supracondylar fractures of humerus in children
Objective To compare the effects of closed reduction and Kirschner wire internal fixation under ultrasonic guidance and C-arm fluoroscopy on supracondylar fracture of humerus in children.Methods A total of 119 children with humeral supracondylar fractures in Children's Hospital Affiliated to Zhengzhou University from October 2020 to October 2022 were randomly divided into a study group(n=60)and a control group(n=59).The control group received closed reduction and Kirschner wire internal fixation treatment under C-arm fluoroscopy,while the study group received closed reduction and Kirschner wire internal fixation treatment under ultrasound guidance.The perioperative conditions(operating time,number of fluoroscopy sessions,blood loss,length of hospital stay,fracture healing time),visual analog scale(VAS)scores,complications,and excellent fracture recovery rate between two groups were compared.Results There were no significant differences in blood loss,hospitalization days and fracture healing time between the two groups(P>0.05).The operation time of the study group was longer than that of the control group,and the times of fluoroscopy were less than that of the control group(P<0.05).At 4-6 weeks and 3 months after surgery,the VAS scores of the study group were not significantly different from those of the reference group(P>0.05).There was no significant difference in the incidence of complications between the study group[3.33%(2/60)]and the control group[5.08%(3/59)](P>0.05).The excellent and good rate of fracture recovery in the study group[83.33%(50/60)]was not significantly different from that in the control group[77.97%(46/59)](P>0.05).Conclusions Both ultrasound-guided and C-arm fluoroscopy guided closed reduction with Kirschner wire internal fixation can promote fracture healing,reduce pain,reduce complications,and improve the excellent rate of fracture recovery in children with supracondylar fractures of the humerus.However,ultrasound-guided closed reduction with Kirschner wire internal fixation can reduce radiation damage in children with supracondylar fractures of the humerus due to fewer fluoroscopy times.
