Application value of high resolution magnetic resonance vascular wall imaging in the evaluation of intracranial arterial dissection at different periods
Objective To investigate the clinical application value of high-resolution magnetic resonance vascular wall imaging(HRMR-VWI)in evaluating intracranial dissection(IAD)at different stages,in order to provide evidence for early clinical diagnosis and selection of reasonable and effective treatment.Methods Seventy-two patients with intracranial artery dissection admitted to Xinxiang Central Hospital from January 2021 to January 2023 were selected,all of whom were confirmed by clinical results.Routine MRI and HRMR-VWI examinations were performed for all patients.The detection rates of lumen and tube wall changes and typical dissection signs at different periods were compared,and the imaging characteristics and quantitative parameters of IAD patients at different periods were analyzed.The consistency and diagnostic value were further analyzed.Results The detection rate of lumen,wall changes and typical signs of intersections at different periods by HRMR-VWI was higher than that by conventional MRI.The quantitative parameters of IAD in acute stage were higher than those in chronic stage and subacute stage(P<0.05).The quantitative parameters of IAD in chronic phase were lower than those in subacute phase(P<0.05).The detection rate of IAD patients at different periods assessed by HRMR-VWI was 92.31%in chronic stage,91.67%in acute stage and 95.24%in subacute stage.The missed diagnosis rate was 7.69%in chronic stage,8.33%in acute stage and 4.76%in subacute stage.The Kappa value was>0.7,indicating a high consistency.Conclusions HRMR-VWI technique has a high detection rate and a low missed diagnosis rate,which can effectively evaluate cranial artery dissection and provide a reliable basis for clinical disease staging.
